Dear sir/madam

I am Madhab Raj Paudel, born on 22 Dec 1974 in kaski, Gharmi, 07 Lamachaur, Nepal; holding the
passport number 08338244.

I am writing this letter as remonstration for the refusal of my visa application, I received from embassy of
poland New Delhi dated 23 April 2018. My visa application was refused for the reason that, "in the
procedure for issuing a national you have submitted an application containing false personal data or false
information or you have attached document containing such information."

To my belief and knowledge, I have found only the false personal information is marital status which has
been mistakenly submitted as single in my visa application form.

I hereby, would like to confirm that my married status is married. In my passport next of kin is chandra
kala paudel, who is my wife, which supports the fact that my marital status is married.

There is also other false information in the visa application form that is the intended date of travel to
poland and intended date of departure from poland, which is rescheduled to 12 May 2018 and 11 May
2019 respectively.

I hereby, submit the correct personal data and informations in this letter, that had been mistaken in visa
application form causing my visa refusal. I have also attached the copy of passport, which the
information that my marital status is married, the official Work permit, accommodation and guarantee
letter from the employer on this appeal.
