1.2 Rationale
Why it is important to learn this material?

A well organized and communicated workplace safety model helps


supervisors and employees be more consistent and thorough in their
approach to workplace safety. In addition, the employees must learn to
think through their work processes and plan for hazards. They must be able
to evaluate their work situation and assess the existing or likely workplace
hazards and the resulting risk to workers, then create as safe a work
environment as possible, and report any hazards to a supervisor.

When complete this material, you will able to…

1. Recognizing Workplace Hazards


“To be forewarned is to be forearmed.” Hazards cannot be
prevented if they are not recognized. At this point, decisions such
as locking and tagging out electrical circuits and equipment should
be made.

2. Evaluating the Danger of Hazards


Once hazards are recognized, workers must evaluate the risk of
injury from each hazard. Never assume the risk is low until the
hazard has been evaluated (“Do it first and ask forgiveness later”
doesn’t work when you’ve been electrocuted!) Because the
conditions at a job site are in a continual state of change (weather,
workers, tasks being performed, tools and equipment being used,
etc) , hazards must be continually recognized and evaluated. To
evaluate a workplace hazard, consider how close a worker or metal
conduit could come to that hazard, consider whether there are
combinations of hazards present that would increase the risk to
workers, and consider any clues to existing issues.

3. Controlling Hazards
Once any electrical hazards have been recognized and evaluated,
they must be controlled. This can be accomplished by either
changing the hazardous part of the work environment (preferred
method) according to OSHA regulations on electrical safety and the
NFPA 70E and/or implementing safe work practices to lessen or
eliminate the hazard.

1.4 Laboratory Objectives


Here is what you will able to do when you complete each
objectives

One way to implement these three components of the workplace


safety model is to conduct a job hazard analysis (JHA) for a
recognized hazardous task or to fix a known hazard. Breaking the
process into the three components of the safety model in chart form
simplifies the safety process, lessening the likelihood that a worker
will expose themselves to the hazard.

Learning Objectives

Here is what you will be able to do when you complete each objective.
Identify, Assess and Control safety hazards in the Wellsite facility

Introduction

Throughout the Wellsite facility you will find many safety hazards. Your first task in
this exercise is to conduct an inspection of the Wellsite facility to determine how
many obvious safety hazards you can locate.

EXERCISE

All jobs need to be assessed for hazards. Since you will be operating this facility it
will be necessary for you to conduct an audit of the Wellsite to become aware of
potential safety hazards associated with the operation of the equipment in the
Wellsite. You will also have to rate the hazards using the method shown.

If you find a hazard, elimination of the hazard is your first choice of action. However
if this is not possible, then control of the hazards by using the hierarchy of controls is
necessary. Hazard control methods are summarized below.

HAZARD CONTROL

The assessed hazards must be eliminated or controlled. If the hazard cannot be


eliminated, controls must be implemented to reduce the risk. In determining
measures to overcome the hazard there are three types of controls to consider:
Following are examples of each type of control to assist in the selection. The
hierarchy of controls must be considered in each circumstance and in the following
order:

1. Engineering
2. Administrative
3. Personal Protective Equipment.

Engineering controls:

 Design of the workplace


 Automation and control systems
 Machine guard, interlocks, lockouts, warning devices
 Isolation/enclosure
 Limitation (safety valves)
 Ventilation (general dilution/local exhaust)
 Air monitoring devices
 Communication devices

Administrative controls:

 Substitution of a less toxic product


 Policies and procedures
 Training
 Organizing and planning work
 Safety plan/procedure

Personal Protective Equipment (PPE)

 Hard hat
 Safety boots
 Eye protection
 Hearing protection
 Fire resistant clothing

Possible hazards you may want to consider when evaluating your operations in the
Wellsite:

RATING POTENTIAL HAZARDS

Once the hazards associated with the jobs/tasks are determined you will have to rate
the hazards as follows:

1. Rank the frequency as per the Hazard Rating chart (see page 4) as follows:
1 - rarely, a person is exposed to the hazard less than once a month

2 - often, a person is exposed to the hazard 3 or less times a week

3 - daily, a person is exposed more than 3 times a week or every day

2. Rank the probability of occurrence as per the Hazard Rating chart as follows:
1 - unlikely to occur

2 - some chance/probable

3 - will occur if not attended to/likely

3. Rank the severity as per the Hazard Rating chart as follows:


1 - first aid or minor property damage, a person administers first aid to self

2 - injury results in lost time, seeking medical help or moderate property damage

3 - injury results in permanent disability, serious health effects or major property


damage

Determine the ranking for each hazard using Frequency + Probability + Severity =
Risk Rating

High (7-9): means the hazard must be attended to immediately. Controls must be in
place and a safe job procedure should be developed and communicated to those that
are affected.

Moderate (4-6): means the hazard requires attention. Controls should be put into
place and safe work procedure should be developed prior and communicated to those
that are affected. Employees must be aware of the hazard.

Low (1-3): means the hazard requires attention. Controls must be in place and a safe
job procedure should be developed and communicated to those that are affected. A
safe work procedure may also be developed.
