AFFIDAVIT OF LEADERSHIP

I, Hans Marc C. Simplicio, Filipino, of legal age, and a resident of Bo. Obrero,
Davao City after been sworn to in accordance to law hereby disposes and say:

1. That I am the President of Youth For Christ of the University of

Southeastern Philippines, Iñigo St. Bo. Obrero, Davao City.
2. That I am a 3rd year student under the program of Bachelor of Science in
Electronics and Communication Engineering.
3. As part of my Duties and Responsibilities, I shall be liable for the official
actions of the said organization during my incumbency.
4. I am executing the affidavit to attest to the truthfulness of the foregoing, as
a Requirement for the (renewal of) recognition of organizations by the
University of Southeastern Philippines and whatever legal purposes it may
serve.
