AFFIDAVIT

I, ROMEL B. MANGUNDAYAO, of legal age, Filipino and resident of Blk 14 Lane

41, Lot 6 & 8 Ph 2, Windward Hills Subdivision, Dasmariñas City, Cavite, after being
duly sworn to in accordance with law, depose and state THAT:

1. I am the son of REYNALDO C. MANGUNDAYAO who is the son of LORENZO

MIRANDA MANGUNDAYAO, whereas he is also the son of SPS. SIXTO
MANGUNDAYAO and SOFIA MIRANDA, my great grand parents;

2. That BARTOLOME MIRANDA MANGUNDAYAO is the brother of my

grandfather LORENZO MIRANDA MANGUNDAYAO, who is also the son of of
SPS. SIXTO MANGUNDAYAO and SOFIA MIRANDA, my great grand parents;

3. That GERRY S. MANGUNDAYAO is the son of BARTOLOME MIRANDA

MANGUNDAYAO, who is the cousin of REYNALDO C. MANGUNDAYAO,
my father.

4. That I hereby under oath attest that GERRY S. MANGUNDAYAO is my uncle.

5. That as evidence of my legal relation to the above said persons, certain documents
are herein attached; and

6. That I am executing this affidavit in good faith to attest to truth for

whatever legal purpose it may serve.
