    2. Admit Card

    3. Result & Rank letter issued by NEET

    4. Class 10th Pass Certificate

    5. Class 10+2 Pass Certificate

    6. Class 10+2 Mark Sheet

    7. College Leaving Certificate / Transfer Certificate from the last Institute studied.

    8. Conduct / Character Certificate.

    9. Category Certificate (SC/ST/OBC/SEBC/PH) from competent authority.

    10. 2 nos. Colour Photographs

    11. Admission Fees as admissible. (Mode of payment will be informed on the day of
