Sir/Madam:

This is concerning CARLITO JALBO, a consumer in Barangay Songkoy of the

Municipality of Kitcharao. His residence has been transferred to (new site) due to
easement relating to river (river name) on (date of transfer). Concomitant to such
relocation is the disconnection of his residential electric line with such
understanding that the same will be reconnected in his new residence.

However, since then and allegedly despite compliance with the requirements,
ANECO still failed to cause the necessary reconnection.

It is with this view that I request your office to shed light on this seemingly deliberate
failure on ANECO’s part to effect the needed reconnection of the electric line of
CARLITO JALBO’s residence.
