Case No.

2:

1. What concerns do you have based on review of the patient’s medication profile?
 According to the patient’s medication profile, he has been taking Glipizide and has been refilled 2 times, it
seems that the patient is not compliant with the dosage regimen, it is said because the quantity given and
the days doesn’t seem to be equated (unless special instructions was given by the physician). The patient
has no known allergies thus won’t interfere with the antibiotic ordered. There are no additional information
listed. The patient medication profile should be updated.
a. What else would you like to know about your patient?
 Is there any progress or problems regarding his disease?
 Are there problems regarding his medications?
 Is the patient currently working?
 Is the patient compliant/ adhering with his medications, diet and exercise regimen?
 Aside from the drugs in the medication profile, what are other drugs (nonprescription drugs) that he
has been taking?
 Aside from his diabetes and cut on his arm, are there any other health problems that he’s
experiencing or feeling?
b. Before talking with the patient, what functional and emotional barriers would you expect during education?
 Functional Barriers:
Based on the patient’s information, he has a construction company thus may have hearing difficulties due
to the noise that he encounters in his work place and his age may also add up in the deterioration of vision and
hearing which makes it difficult for the patient to absorb information during the consultation..
 Emotional Barriers:
It is said that the patient is “on the go” (very active or busy) thus may result for the patient to give the attitude
of a “know-it-all,” seem unmotivated or uninterested due to his work that will interfere with the effectiveness of
counseling or interviewing and, more important, impact the patient’s decision to comply with therapy.
c. What are the goals of the education?
i. To attend and inform the patient regarding his infected cut on his arm
ii. To inform the full directions of use of the given drug in the prescription- cephalexin
iii. To monitor, assess and improve therapeutic and non-therapeutic medication use regarding
patient's condition-DM
iv. To correct medication therapy as well as to promote and monitor patient adherence
