PARENTAL CONSENT

I/We hereby willingly and voluntarily give consent the participation of my/our daughter
Irene Malnegro in the SSG Federated Elections and Capability Building Seminar to be held at Alta
Bohol Resort, San Isidro, Baclayon, Bohol on July 2-3, 2018.

I/We have considered the benefits that my daughter will derive from her participation in
this activity provided that due care and precaution will be observed to ensure the comfort and
safety of my/our daughter and that DepEd employees and personnel may not be held responsible
for any untoward incident that may happen beyond their control.
