I / We am/are registered with Motilal Oswal Securities Ltd. (MOSL) as a client for execution of trades
from time to time in Segments like Capital Market / Equity Derivatives / Currency Derivatives Market
of Bombay Stock Exchange (BSE) / National Stock Exchange (NSE). Also I / We have demat account
maintained with MOSL.

I / We wish to commence trading in commodity futures / options / options on commodity futures /

derivatives segments of MCX and NCDEX for which I / We am / are providing authorization, as under,
through this consent:-

This authorization is in addition to any authorization given earlier by me/us.

1. TRADING PREFERENCES

I/ We have ticked the relevant boxes, reflecting the Exchange where I wish to trade in commodity
futures / options / options on commodity futures / derivatives contracts. The Exchange not chosen is
struck off by putting the mark “X”.

2. I / We confirm that all details including correspondence address, contact details, income
details as submitted in the KYC or any time thereafter modified by me, in MOSL records are
correct as on date for any communication with me / us. In case of any changes therein, I
undertake to update the same immediately.

3. I / We confirm having received, read and understood the contents of the Standard
Documents i.e. Rights and Obligations document(s), Uniform Risk Disclosure Document, Risk
Disclosure Document for Options, Do’s and Don’t’s for Investors, and also other documents
i.e. Voluntary Clauses, Policies & Procedures etc. which are also made available in my client
login for my understanding for dealing in Commodity Segment i.e. MCX and NCDEX. I / We
have also been informed that the said standard set of documents as mentioned above has
been displayed for information on the Website of Motilal Oswal Securities Ltd. I/We do
hereby state and declare that I/we have read the same and understood the contents there
of and shall bound by such provisions as outlined in these documents.

4. I/We hereby give my consent to apply the below mentioned brokerage to my transactions.

5. I/We understand that my Client Code will remain the same & all trades in MCX and NCDEX
(in all segments) will be done using the same Client Code.

6. I / We understand that the Know Your Client (KYC), mandatory documents and voluntary
documents like Power of Attorney, Running Account Authorisation, Risk Profiling
Questionnaire, Family Declaration, Authorisation for Trading Account, FATCA Declaration,
Nomination Form etc. where ever applicable, executed at the time of registering me/us or
thereafter as a client under the above client code, applies mutatis mutandis to trade(s) in
MCX and NCDEX (in all Segments).

7. In term of executed POA, if any, for ease of operations, I / We authorize you to use the
shares lying in my existing Demat account (s) held with CDSL / NSDL
as mentioned in the POA mapped to my / our trading account towards margin / pay-in or
settlement obligations / collateral and such other provisions as mentioned in the POA, for
trading in MCX and NCDEX (in all segments).

10. I/We give my consent to send Contract Notes via electronic mode on my registered email id
in MOSL records. I/ We are aware that non- receipt of bounced mail notification by the
member shall amount to delivery of the contract note at the above email ID.
 11. I/We understand that, for the purposes of meeting the requirements mentioned in point
number 7 above, securities lying in my demat account can be transferred to following demat
accounts of MOSL in addition to existing demat account of MOSL mentioned on POA document
or updated thereafter under intimation to me:-

13. I / We hereby urge you to transfer my / our outstanding position if any, from Group company Motilal
Oswal Commodities Broker Pvt. Ltd. (MOCBPL) to Motilal Oswal Securities Ltd. (MOSL) at prevailing
prices. This will help me / us to avoid unnecessary cost in lieu of such transfer. I / We shall fulfil my /
our obligation to MOSL for such positions transferred.

I/ We request you to consider this application and enable me to deal in above said Exchange(s) /
Segment(s).
