Further to your enquiry made under the provisions of the Freedom of Information Act 2000, which
we received on 26th January 2015, you requested the following information:
Please provide a copy of the latest Patient Report Form Template (that reflects the
changes that have been made to the PRF) in addition to a copy of the previous
version of the Patient Report Form Template (i.e. prior to the most recent
changes).
The current version of the Patient Report Form (v4) is attached as is the previous version in use.
We hope that this answers your request. If you are dissatisfied with our response you have the
right to seek a review in line with guidance from the Information Commissioner.
In the first instance, this should be addressed to me at the above address. I will provide a response
within 20 days of receipt.
Should you remain dissatisfied with the outcome of this review then a final review request should
