Dear Sir / Madam,

This letter is to request to your good office to divide by two the medical expense paid from my
hospitalization last August 9, 2018, thursday afternoon which was the total amount of Five Thousand
Five Hundred Pesos (P5,500.00) . I just collapse inside the office during office hours due to acidity
which shows from the medical results in the hospital. I would like to request to charge me only the half
of the total medical bill amounting Two Thousand Seven Hundred Fifty Pesos (P2,750.00) instead
because the bill was too expensive for me to pay all, as of the moment I am still in minimum wages . I
was not yet enrolled in Maxicare but I almost a year serving the company this coming October 2018
and the HR said that they will going to enroll me in Maxicare this August 19, 2018 because it was the
day of renewal of Maxicare.

I would like to pay the said amount (P2,750.00) two times every cut-off through salary deduction.
