SUBJECT : Request for Transfer of Branch (CF – Malate to SOG 1 Winford Tayuman)

To Whom It May Concern:

I would like to request your kind attention to my application for a transfer of work location from
PAGCOR CF Malate branch to PAGCOR SOG1 Winford Tayuman branch. This is due to some
unavoidable personal reason.

My wife and I decided to settle here in Manila many years ago. We also decided that my wife will
temporarily stop working to take care of our only son considering that we have no one to leave our
son since my family is in Oman and my wife’s family is in Laguna. We have now our life here. My
5year old son is presently studying at St. Vincent Learning Center here in Sampaloc, Manila. We
take turns in taking care of our son. For the past few months my wife has been suffering from
headache/migraine most of the time, asthma and allergic rhinitis which requires a constant check-
up. Her medication indicates the side effect of dizziness. My wife alone cannot handle an all day-
and-night care. I take shifts with her and we take care of our son together.

Moreover, I have been working in our company for a decade now and I have been loyal to our
organization during my tenure here. I sincerely wish to continue my association with the company
and gain valuable experience in a new location which is more convenient for me so that I can assist
my wife to take care of our son and take care of her as well during my free time. This transfer is
really important for me and to my family.

I humbly request you to transfer me from CF Malate branch to SOG1 Winford Tayuman
branch because it is more nearer to our home so that I can attend to my family’s needs easily for I
know that it is during this time that my wife needs me the most. Please, try to understand my position
and relieve me accordingly. I shall be very thankful to you.
