1. THAT I am an active BJMP personnel, and included in the NHA Alpha list of
qualified applicant/awardee under the National Housing Authority (NHA)
AFP/PNP/BJMP/BFP/BUCOR Housing Program;

2. THAT during the NHA process of identifying and choosing qualified

applicants and through my application, I was pre-qualified, approved, and
allocated a real estate property from the NHA Housing Program particularly
described as Block No. ___, Lot No. ___, consisting of forty (40) square

3. NOW THEREFORE, being a NHA AFP/PNP/BJMP/BFP/BUCOR Housing
