Brief History/Background:

Liquigaz Philippines Corporation was established in 1995 and is now the second largest LPG supplier in the country with more
than 30% of market share. We are owned by Canaria Holdings Corp. and headed by our President & CEO Mr. Lucio L. Co.

Problems Encountered and Solutions made


i. A Lorry was overfilled because an operator didn’t notice the Magnetic Gauge’s value (volume.) was
already at 95% which was supposed to be just at 89%.

 The Operators managed to solve this problem by closing the liquid valve connected to the
overfilled Lorry. They did a line-up of valves to be able to connect the outlet of the overfilled
truck to the inlet of another truck. This is for transferring the excess amount of the product to
the other Lorry.

ii. An outlet inside the Conference Room exploded due to a Short-circuit.

 We immediately used fire extinguisher but when we saw that the smoke was still there, our
Safety officer, Engr. Raynel Rodriguez, pulled down the circuit breaker and reported it to the
MCR. But since it was already past 5pm that time, the Maintenance team wasn’t available but
the next day, they managed to fix it.

iii. A Lorry’s Tank became over pressured while the process of Truck loading is occurring. As the Road
Loading Arm was connected to the outlet manifold of the Lorry, the vapor hose must be connected
to the vapor manifold for the vapor return but the Operator didn’t notice it, that’s why the Lorry’s
Tank was over pressured which caused the liquid not to enter the Tank because of the pressure
inside it is higher than the pressure the Liquid product is carrying.

 The operator closed the liquid valve and connected the vapor hose. He let the vapor return flow
through the hose as a Vapor return and for the Lorry to lower its pressure. The Pressure of the
tank to be achieved is at about 7Barg. When it was already reached, the operator re-opened the
Liquid valve for the process to continue.

iv. While there was an importation of product, the Mechanical Seal of one of the Booster Pumps being
used was damaged which caused the product being imported to leak out and spout.

 The Booster Man, who was the assigned operator on the booster area, immediately shut
down the Booster Pump that leaked. The Maintenance team couldn’t change and
replace the Mechanical Seal until the new one had arrived and delivered by the team of
EAGLE BURGMAN. Fortunately, no one was hurt during that incident.
