Dear Parent/Guardian,

PARENT/GUARDIAN INFORMATION AND CONSENT FORM FOR SCHOOL INCURSION

We are pleased to provide you with the following details regarding our upcoming incursion visit from
REmida Perth, on Monday, 22nd of October 2018.

REmida is a local run company that collects recycled materials and teaches the community how these
products can be reused to reduce waste.

This excursion has been planned to supplement the work being completed in your child’s classroom in
regard to the Design and Technology curriculum, focusing on the strand of Processes and Production
Skills. This excursion will also consolidate their learning about Sustainability.

Cost

The cost of the excursion is $15 per student, this will cover the cost of the REmida Educational
Workshop. This cost is included in your term 4 account.

Supervision

Miss Koncurat, Miss Steepel, Mr Osborne and Miss Abbonizio will be the supervising teachers during
this incursion, two parent volunteers would be appreciated for help during the workshop. This will allow
for all students to be supervised and one teacher to float between the student groups.

Emergency Contact

In case of an emergency during the incursion, please contact the school on 08 9222 1234. The school
administrator will contact Miss Koncurat, Miss Steepel, Mr Osborne and Miss Abbonizio.

Activities Undertaken

- Students will take part in the Junk Bug 2.0 Workshop.

- Create a sculpture out of recycled materials.
- Learn about sustainability and waste.
- Use critical and creative thinking to solve problems they face during this workshop.

If you have any questions in regard to this incursion or would like to volunteer as a parent helper, please
speak to your child’s class teacher. Please sign and return the attached permission slip. We look
