1. Toolbox Talk Tour/Site Visit (En. Ghazali)
- Most rotating equipment is housed (protected by a fixed structure - because of weather condition)
- The recently commisioned PCP (Pengerang Co-generation Plant), have 4 massive gas turbines, of which 2 of them
are already running. SIEMENS was responsible for the construction of the PCP.
- Turbine (regardless gas or steam type) is only the prime mover, the shaft will transmit the kinetic energy into any
load e.g compressor & power generator
- The purpose of the PCP is to supply electricity and power to PIC, and the surplus will be sold to TNB (Tenaga
Nasional Berhad).
- concept of PCP (gas turbine) : External motor is required during the start of the operation to compress the air-
fuel mixture. Once temperature is self-sustaining for combustion to occur and drive the compressor, external
motor will be shut down. The gas ignited causes fluid (air) temperature to rise, essentially expanding, producing
hot jet stream in constricted flow, turning the shaft and drives the compressor to draw in more air, sustaining the
continuous combustion. The surplus of kinetic energy will turn the generator turbine blades which in turn
produces electricity. On the other hand, the waste heat from the gas will be channeled to waste heat boiler in
order to produce HP steam from Demin water. HP steam can be step down further to satisfy demand of MP, LP
and LLP steam.
- This is why it's called Co-gen. Cuz there are 2 types of boiler:
1) Waste Heat Boiler
2) Auxiliary Boiler

1. Management walkabout with Mike
- Went for a site visit at P6A (LLDPE Project) by Samsung Engineering
- Divided into two separate groups, where each group went to separate section of the site.
- Mike emphasised that this walkabout is not about finding and putting blame onto anyone, rather, to provide
assistance to the contractors.
- Throughout the walkabout, Mike has demonstrated exceptional level of TELL ME, NURTURE TRUST and SHARED
SUCCESS through face-to-face engagement with general labor of multiple nationalities. A few souvenirs were given
to the general labor as token of appreciation for displaying knowledge and awareness on safety.
- Inspection tag in wrong color/ inspection yet to be conducted.

- Presence of translator reduces language barrier
- One worker was found working with torn jean. This may pose hazard, as it may get stuck while climbing up/down
scaffold. However, he was not working on the scaffold that day.
- To prevent tripping and falling hazards (scaffolding tubes): barricade and signage
- Housekeeping
- Work limit - engineering design document. One pipe spool positioned on top of stands was not labelled with load
capacity. How to ensure the stands can withstand the load if the weight of load is not known/labelled ?
- Condition of PPE. Worker does not know when to replace PPE in case of visible crack/fatigue.
- "Comply and Intervene" with demonstration. Prize given to workers managed to answer the questions.

1. Specialist (Bus Inspection)
- There are about 81 white buses (BB, standing bus type). Each can accommodate 95 passengers at one time.
- Bus is manufactured by SKS. Specifications are provided by PETRONAS based on SPAD requirement.
- Since seatbelts are compulsory as per PTS requirement, deviations were deliberated by Group Technical
Authority (GTA) specifically for RAPID project since most of the passengers are going to stand during the journey
to/from work. Management of Change (MOC) was initiated as part of the risk assessment and concluded that at
35 km/h, risk of hazard is kept at minimum, hence the installation of electronic speed limiter.
- 5 buses were inspected using HSE HITS checklist. Part of official documentations were required to be displayed at
windscreen, and part is kept inside the bus.
- General bus requiremnets, such as seatbelts (for driver), windshield, rear-view mirrors, electrical system, bus
enginer and tyres, first aid kits, fire extinguishers, breakdown triangle and reflective sticker.
- Bus operators faced challenges as some of the workers steal the bus emergency window hammer breakers, while
some lean on the emergency windows (breaks easily), causing it to accidentally break.

2.1. Non-accidental death
Storyline: Came back UOIJV camp, went to JP for personal business, back from JB and rest, DP found unconscious
at dinner time, CPR by DFA
2.2. Suspected food poisoning case
Story: involved 28 phillipines. Still in inestigation. Many contradict points.
- Most of the IP went to toilet in the midnight but they didnt remember but each of them didnt meet one another.
- IPs went to work and visted the clinic after the toolbox talk instead of going to the clinic directly.
- IPs did not went to toilet once in the clinic.
- Few IPs said they di vomit at night but they said they didnt in the next intervew.
- Other workers ate the same food but didnt suffer from food poisoning, the IPs are all phillipines.
- IPs having salary problem with contractor and requested for phillipines food to be served before.

Scenario: PMC Health & Medical Manager received an alert from SMF of Samsung camp that cluster of workers
with sign of Influenza XY. The total no. of infected workers will be 20 persons.

To increase PIC capacity to deal with a pendemic and to raise awareness among key persons. The main purpose of
this tabletop exercise is to delay the rate of infection and to protect PETRONAS' reputation (as per verbal
communication with Dr. Mazlan).

3. HSE Center Directorate meeting with Mike - awareness

1. Confined Space Entry
- Stand-by person and fire watcher can be the same person, depending on situation. Stand-by person is
responsible for the communication with the entry person but is not allowed to enter the confined space. A
standby person has to contact relevant party in case of an emergency. Fire watcher is responsible to put off any
fire and allowed to go into the confined space if it is big enough to fit the entry person and him. A fire-watcher has
to continue look after the work until it is cooled down though the work is stopped.
- Ensure clearance in the entrance to allow exit.
- 4 tests: O2, LEL, toxic gas (CO, NO2) with a similar gas tester, adjust K factor.

7. Central Emergency Fire Services (CEFS) Briefing (En. Fikri)


- Purpose: field response for the fighting, rescue, inland oil spill, road traffic, HazMat, and special service type of emergenc
- Current facilities ( 1 water fire truck, 1 foam fire truck, 1 MRV, 2 RIV). Both trucks have the capacity of 12,000L water / foa
concentrate.
- Facilities to be improved to 21 truck in the future
- CEFS response to incident for both inside and outside PIC
- Information relay from public/staff through emergency contact number provided, information will be passed to RAPID
Security Control Centre (RSCC), and to then to Operation Control Centre (OCC). OCC will then activate CEFS response team.

CEFS PIC 6 training modules: HAZMAT, breathing apparatus, basic fire protection system, rope rescue, basic industrial fire
fighting and truck loperator training.

4. INFO - Patuh dan Tegur (Dr. Muthu)


- Dr. Muthu demonstrate Patuh dan Tegur by intervening one of the contract workers who was wrongly carrying the cable
wrapped around his head. The correct action is to put it on the shoulder and not wrapping it around the head.

4. Specialist (Electrical Safety Inspection)


- Visit to Main Control Building (MCB). All process and power control is controlled in a centralised facility. Information feed from all plan
section is sent through underground Fibre Optic Cable (FOC).
- DCS for refineries, crackers, and petchem(process) are separated from utilities.
- During pre-commisioning period, temporary power is supplied by 2 Portable Electical Equipment and Temporary Installation (PETI). T
are essentially a generator.
- Various stringent requirements were placed to ensure safety of the equipment. Among others were the presence of certified Electric
Man, Generator Protection Relay (incase of current leakge), fire extinguisher and certification/approval from DOE (cuz the generator
generates byproduct of >50kg/h from combustion)
- MCB is equipped with Uninterrupted Power Supply (UPS) system. In the event of power interruption, backup power (stored in the for
batteries) will supply electricity with capacity of 8 hours. UPS will be separated into two clusters, one room each. both rooms will place
different capcity of battery to suit different types of equipment.
- Training room (simulation room) is available at site.
- Exit and hazards signs are clearly displayed at various and strategic location at site.
- Visit to substation and Field Auxiliary Room (FAR).
- Main function of substation is to step down voltage from main power station (PCP) to lower voltage suitable for equipment plant use.
- All process unit will have their own FAR and substation. Every processing equipment will feed information first through Junction
Box/Board (JB), then into FAR, and finally to MCB.
- Ground color (red): electrical wire underneath and Ground color (green): instrument wire underneath
- Cable color: red (transmitter - trip, for safety interlock), blue (transmitter, cant trip, more to measurement and monitoring intrume
black (normal, e.g. motor), green (earth)

2. Specialist (Lifting operation)
- Inspected 2 type of crane: Mobile crane and crawler crane
- All drivers need to have the proper license and qualification in order to be competent to operate the crane (RAPID Pass, C
Competency slip)
- Load charts is the schematic or graphical chart which can provide the amount of load allowable for specifc length of boom
arm and working radius.
- If a lifting work requires additional working radius, an additional extension (jib) can be added to increase the working radi
however the amount of load that can be lifted is significantly reduces.
- Rigging and fitting activity can only be executed by trained personnel who have undergone training
- Daily crane inspection needs to be done by the crane operator following a set of checklist specified to the type of crane
- Load Moment Indicator is a system which aids the crane operator by sensing (directly/indirectly) the overturning moment
the equipemnt (load multiplied by radius),
- In the event of a crane lifting over its capacity or over hoist, LMI will trigger the light alarm, followed up by sound alarm an
then shut off power to equipment functions that can increase severity of loading (i.e. hoisting). However, function that
decrease severity remains operation (i.e. lowering)
- Sling used for lifting is color coded
- Tagline is used to maneuver the load to the designated place.

2. Social Performance (SP) Tier 2 Audit at Punj Lloyd (as observer)
- 2 execs and 1 manager (Joanna) in SP HSE Central Directorate (CD). Camp Audit will have their own Subject Matter Expert
(SME) in various subjects, and from various consultation companies to maintain neutrality. PETRONAS as project owner,
observes, oversees and ensure work is done by the consultant. Most of the worker in Punj LLoyd camp (P-22) is from mainl
India, Pakistan, and Bangladesh. Audit is conducted ever 6 months.
- The team convenes in main meeting room at site and were divided into 4 subdiciplines (Food Quality & Hygiene [FQH],
Health Safety and Environment [HSE], Operation & Maintenance [OM], and Security).
- The team went to the manager's office in the kitchen, and scrutinise the official documents pertaining workers' vaccinatio
for typhoid, medical record). A few of the expiration dates of the typhoid injection were wrong.
- Hazard Analysis and Critical Control Points (HACCP) is widely used to control food quality by attempting to avoid hazards
rather than inspect finished products for the effect of those hazards.
- Storage area for kitchenware were checked. A few of items were not in their correct storage compartment. Kitchen utens
were not supposed to be placed together with food items.
- For every meal, sample of the raw meat/perishable ingredients must be stored for up to atleast 3 days in the freezer and
must be labelled accordingly.
- Monitoring station for pest control must be adequate.
- Procedure for thawing of meat must be correct.

3. CCSM (Package 1) site visit (En. Hisam Hashim)


- CCSM area (400m x 500m)
- Consist of 4200 workers (mostly chinese)
- Divided into 3 area (Common area, RCC1 and RCC2)

4. Security compentency inspection on alcohol blowmeter usage(Mr. Reeziz)


- Complaint from workers regarding security staff at the gate puts the alcohol blowmeter too close to the mouth, causing
hygiene issue.
- Inspection at the gate shows that security staff are competent and understand how to use the alcohol blowmeter.
- Arising matter: inadequate alcohol blowmeter during peak time (festive, payday), workers does not cooperate with the
security (does not blow to the blowmeter)
- Way forward: to request more alcohol blowmeter to be placed at the gate

- Split into two teams for inspection in two buildings each.

Problems found:
- Malaysian (SMQA) complained food is dry. Request food comes with more sources and gravy
- Water disruption - 10-4pm.

- Vietnamese (MQC) complained about food hygiene. Worm was found in food.
- Inconsistent salary pay.

- Indonesian (KQS) complained about many mosquitoes.

3. Environment (Debra & Hamka)


- Environment Inspection on waste management
- Environmental inspection is done twice per week at site. The main focus area that the team are currently looking at is the
management of waste, soil erosion, and dust (erosion and sedimentation control)
- Visit was conducted at P-14, mainly on the construction/installation of drainage system.
- Food waste, housekeeping of wastes around the site is mainly done by SJ varied.
- Preventing soil from entering the drain will reduce maintenance issue and ease cleanup.
- There are issues on the housekeeping material for the used construction materials (broken concrete, pile) where it was le
near the drain. PRPC environment team told them to expedite the disposal of those materials appropriately.
- Quantity of soil in the drain should be kept at minimum during the drain construction since it will decrease the drain capa
and incur post-construction cost needed to remove the soil.
- Check dam helps to reduce the velocity of water flow. Often, gravels, stones, are used to reduce the flow velocity, promot
the disposition of soil/sediment, ensuring water going out from the check dam clear.
- All the drainage inside RAPID project is connected to a single outlet drain with the size of 44m width.
- Slope stablization used in RAPID (gravel put at the edge of the drain to reduce mud from entering the drain)
- Grass is avoided to be used as slope stabilizer to remove the hazards of fire.
- Watering roadsides is one of the method of dust control
