citizen and resident of Pagdaraoan, San Fernando City, La Union
Philippines, after having been duly sworn to an oath in accordance
with law do hereby depose and say that;

1. I am the registered owner of E & M’S KOREAN STREET

FOODS with Certificate Number 04890796, located in
front of DSWD Catbangen, San Fernando city, La union;
2. My permit to Engage Business under Business Permit No.
002036 was issued on January 12, 2017 at San Fernando City,
La Union;
3. I undertake not to utilize shoulder of the road for my business,
E & M’S KOREAN STREET FOODS;
4. In case I would renege from my pledge not to utilize the
shoulder of the road, I am aware for the cancellation or
revocation of the said business;
5. I undertake not to transfer or move my cart to any place other
than that specified business location; and
6. I am executing this affidavit to attest to the veracity of the
foregoing and for all legal intents and purpose this may serve
best.
