Bldg., Edang Street, Pasay City, after having been duly sworn in accordance with law, hereby depose and state:

1. That I am the owner of business establishment A & G SPEEDWASH D.I.Y LAUNDRY;

2. That as a entrepreneur I conduct business at 2035 F. Muñoz St., Brgy. 740 Zone 80 Malate, Manila.
3. That the owner of the property where my business is located is CARMENCITA N. MEER;
4. That Carmencita N. Meer is my close friend. As such, she does not require me to pay rent for the
use of her property;
5. That in view of the my friend’s generous accommodation, I am not paying any rental fee for the use
of her property for commercial purposes;
