    Patient come was a 18 years old, woman. Yesterday she complained about her diet.
    She rarely eat foods ( not appetite ) and said felt nauseous. She told everytime eat something
    her stomach feels sore pack. At half past three in the afternoon she felt like throwing up,
    slack Untill fever. She do the diet about three weeks. As long as diet she had difficulty in
    digesting food.

    NOTES

    Admitted to hospital : Tuesday 25th Sep 07:15 pm

    Presenting symtoms : Nausea, not appetite, stomachache, throwing up, and


    fever

    Diagnosis : Anorexia Nervosa

    Treatment : Stop do the diet, consuming fruit, consuming protein


    food, and reduce consuming carbohydrates

    Follow-up : Wednesday 13:00; No symptom,

    HR (Heart Rate) : 86 x/minutes

    RR (Respiration Rate) : 17 x/minutes
