DELIVERANCE - LETTER OF ACCEPTANCE

Please read carefully and sign below to give your full consent to the ground-rules of our Deliverance Program.

HOURS:
 Day 1 – Friday: 10:00 pm – 2:00 am
 Day 2 – Saturday: 9:00 am – 1:00 pm
 Day 3 – Sunday: 8:00 am – 11:00 am
GROUNDRULES:
 Be punctual, and plan to attend all three days.
 Two-day dry fasting (NO FOOD, DRINKS, WATER, OR CHEWING GUM) starting with last meal on
Thursday before midnight until the end of the program on Saturday.
 No jewelry of any kind for both men and women.
 No make-up or nail polish.
 All women must cover their hair.
 No hair attachments.
 Women must be appropriately dressed: wear long dress or skirt; women wearing trousers will NOT
be allowed.

I agree to indemnify Mountain of Fire and Miracles Ministries (MFM) Bowie, MD,
for any injury, mishap or accident suffered during the program.
