Affidavit of Witness

I, Jonathan Bautista, of legal age, Filipino, with residence and

postal address at Igulot, Bocaue, Bulacan after having been duly
sworn to in accordance with the law depose and say:

1. That I am presently employed at Nature Spring Waters at

Guiguinto, Bulacan designated as Supervisor.
2. That I am the immediate supervisor of Aldrin Cruz, a
warehouseman of the same company.
3. That I was on duty on August 30 and 31 2018.

 4. That on August 30 at around 5:00 in the afternoon, I instructed
Aldrin to render overtime work during the nightshift to
accommodate the high work load.
5. That on August 31 at around 12:00 in the afternoon, I went to
the employee’s area where employee’s are allowed to rest or
sleep during rest periods, to rest.
6. That I saw Aldrin slip and and fall down on top of another
employee which I later identified as Christine Denise Abuel.
7. That I saw Denise kick Aldrin right after the incident.
8. That I am executing this affidavit to attest to the truth of the
foregoing statement and to support the dismissal of the case
filed against Aldrin.
