from 07/02/2013 to 17/06/2016. During this period, her service were found to be satisfactory
in carrying out the job duties, her responsibilities were to;

 Provide direct resident care while promoting resident dignity independence choice.
 Ensure the highest levels of care are maintained by supporting and assisting clients
when required with washing, dressing and all other aspects of daily living.
 Regularly observe residents physical and mental conditions.
 Assist with ADLS: taking vital signs and weights and accurately documenting care service
provide and observations on communication log and incident reports: reporting all
incidents immediately.
 Provide transfer assistance by moving client in and out beds.
 Create and maintain a pleasant work environment.
 Prepare hygienic meals and snacks for resident.
 Housekeeping and laundry.
 General help to elderly resident.
 Companionship and discussion. Light exercise.
