ANNUAL IEP CHECKLIST

 Send out Meeting Notice


 Need at least 3 date offers
 If confirmed through email, print out and attach email to physical Meeting Notice. Okay
if parent signs Meeting Notice at IEP if already agreed in writing via email.
 Prepare Excusal Form if team member is unable to attend/attend in full
 Need Admin signature
 Send home BEFORE IEP meeting
 File Meeting Notice and any email correspondence into Blue File
 Print/Copy Agenda
 Print Procedural Safeguards (Parent’s Rights) in appropriate language
 Spanish & English
 Print/Copy/Update Progress Report on current goals
 Send home BEFORE meeting
 Update dates on Info & Eligibility
 Do NOT change Eligibility statement → Consult with psychologist/speech
 Update Present Levels
 Include current dates on each section, remove old present levels
 Special Factors
 Assistive Technology
 Low tech: pencil grips, adaptive paper, slant board, etc.
 High tech: AAC device, text-to-speech, speech-to-text, keyboards, etc.
 If behavior impacts learning:
 Special Factors Action Plan (SFAP)
 Need a Behavior Goal
 Behavior Intervention Plan (BIP)
 Need a Behavior Goal → Must have an FBA
 Include School Psychologist
 Statewide Assessment: update and print
 New goals: See Goal Writing Training
 Goals for ALL areas of need
 Check Annual Goal and Benchmark Dates (Nov., Mar., June)
 Benchmark and goal criteria match
 Tie all goals to a curriculum/state standard
 Persons Responsible: only person reporting on goal
 Services – Offer of FAPE
 Do NOT update, print out current one and make changes at the IEP
 Services – Educational Setting
 Do NOT update, print out current one and make changes at the IEP
 Parent Consent Form
 Uncheck - “As a means of improving services…”
 Notes
 Take on laptop during meeting on Agenda in Word or Googledoc, then add into SEIS
 Staffing: set up with site team PRIOR to meeting and determine roles
 Facilitator, Note Taker, Timekeeper, etc.
 Look over DIS provider’s progress report to see how student performed
 Look over DIS proposed goals to see if any of these goals could be shared goals
 Affirm/Attest
 Copy of Final IEP: Send home to parent
 Original Draft IEP with changes: Blue File
