Dear Parent / Guardian:

Thank you for your interest in volunteering. Please complete the attached forms and return
them to your child’s school. The district will approve Volunteers only twice a year.
Volunteers need to get their application form and background check completed by September
15th or March 15th prior to being able to volunteer for the district. Volunteers to the District
will NOT be grandfathered in each year. Volunteers must turn in their paperwork prior to
above dates and have background check completed and approved prior to the deadline each
year. The Principal at your student’s school will direct the placement of volunteers in the
building; please contact the school secretary prior to your first visit.

Forms attached to be completed by prospective volunteer:

1) Volunteer Information and Waiver of Liability

2) Disclosure and Authorization Form: This form allows us to submit your information for a
criminal background screening.

3) Acknowledgment of mandated reporter status

4) Acknowledgement of receipt of district sexual harassment & internet use policies

5) Authorization for electronic network access (return last page with signature)

6) Administrative procedures regarding the securing & screening of resource persons &
volunteers (information only)

The work of our volunteers is greatly appreciated and we truly value their assistance as we
work to make all Litchfield schools better learning places for our students. Thank you again for
