1. That on February 14, 2018 at around 8:00 pm, MAREE BAJAMUNDE

requested for a medical assistance and ambulance;

2. That she reported a vehicular accident involving two vehicles, a

motorcycle and an Toyota fortuner, Maharlika Highway, Concepcion
Grande, Naga City, in the vicinity of Carmelite Monastery and Petron Gas
station and with two injured persons;

3. That immediately, I dispatched the ambulance with plate number XYZ 123
and driven by Mr. Pedro Penduko at around 7:23pm;

4. That also with the ambulance is the Emergency Medical Team of the
Public Safety Office.

5. That records show the Emergency Medical Team arrived at the place of
incident at about 8:10pm on the same date.

Remarks;

This Certification is being issued on the request of the interested party for whatever
legal purpose it may serve.
