1. Since no trauma, must exclude joint / soft tissue infection first eg septic arthritis.

Undiagnosed joint / soft tissue infection in the knee can cause further cartilage and bone loss,
extensive cartilage damage can occur within hours of infection onset, may cause sepsis as
well.

2. The major causes of acute monoarthritis include trauma, infection, crystal-induced


arthritis, osteoarthritis, hemarthorsis, systemic rheumatic diseases (SLE, RA, sarcoidosis),
tumor, and mechanical derangement (meniscal tear, osteonecrosis) and problems with nerve
