CONSENT TO PARTICIPATE IN A RESEARCH

I understand that:

 I am invited to participate in a study for a class requirement of Karen Mae M. Argonza and
Mark Angelo N. Burdeos in Sports Science 152 (SS 152): Biomechanics II of UP College of
Human Kinetics under the supervision of prof. Thor Manlangit.
 I will undergo a pretest, do at least 4-6 sessions of the workout program, and posttest.
 This study will bring me no harm and the results will not be used against me in any way.
I agree to participate in this case study conducted by Karen Mae M. Argonza and Mark Angelo
N. Burdeos for their course requirement in Sports Science 152: Biomechanics II.
