Hirapur Dhanbad (Jharkhand) hereby solemly affirm and declare that I am the owner of the property
situated at “S/o Mishri Lal Yadav, Simla Bera Dhaiya, Hirapur Dhanbad (Jharkhand)” and paying
the all the statutory dues, taxes, bills related to the property mentioned here. The copy of Electricity Bill
is attached herewith.

That, Anubhav Kumar Sah (Proprietor: 69Online Shopping) is having his Place of Business at same
premises mentioned as “Ram Rup Pd Singh, S/o Mishri Lal Yadav, Simla Bera Dhaiya, Hirapur
Dhanbad (Jharkhand)”.

I hereby declare my consent in favour of Anubhav Kumar Sah for the purpose of establishment of proof
as place of business for registration under Goods and Service Tax, 2017.
