Nursing Care Plan

Nursing Diagnosis:

Activity intolerance r/t imbalance between oxygen supply and demand and sedentary lifestyle aeb
fatigue, weakness, and exertional dyspnea

Defining Characteristics:

Fatigue, weakness, exertional dyspnea, nasal oxygen

Patient Expected Outcome:

o Pt will have an increase in activity tolerance by discharge.


o Pt will be able to decrease nasal O2 to 3L by discharge.

AEB:

 Decrease in fatigue
 Decrease in weakness
 Decrease in amount of nasal O2
 Increase in tolerance to normal activities

Individualized & Prioritized Nursing:

1. Mobilize the pt as much as possible.


2. Observe and document skin integrity daily.
3. Monitor respiratory response to activity-look for increase SOB, loss of ability to control
breathing rhythmically, nasal flaring, distress, skin tone changes.
4. Instruct and assist pt in using conscious, controlled breathing techniques during exercise such as
pursed lip breathing.
