- คัดจมูก น้ามูก จาม คอแห้ง อาจเจ็บคอ ปวดเมือ ่ ย อ่อนเพลีย ไข้ ไอ
- มักมีสาเหตุจากไวรัส
- ไม่ควรใช้ยาต้านจุลชีพ ไข้มกั หายใน 3-4 วัน เจ็บคอ 4-5 วัน น้ามูก 5-7 วัน ไอ 7-1
- การใช้ยาต้านจุลชีพไม่ลดความรุนแรงและยะเวลาของอาการดังกล่าว
คอหอยอักเสบ/ทอนซิลอักเสบเฉี ยบพลัน (acute pharyngitis, acute tonsilitis)
- เจ็บคอ กลืนเจ็บ มักมีไข้ อาจมีอาการโรคหวัดร่วมด้วย
- ตรวจพบคอแดง (injected pharynx) ส่วนมาก (>80%) เกิดจากไวรัส
- พิจารณาใช้ยาต้านจุลชีพในผูป ้ ่ วยทีม ่ ีลกั ษณะต่อไปนี้ตง้ ั แต่3 ข้อ คือ 1) ไข้(อุณหภูม> ิ
2) Exudate/Pustule ทีค ่ อหอย/ทอนซิล 3) ต่อมน้าเหลืองทีค ่ อ (Anterior Cervica
Lymph Nodes) โต/กดเจ็บ และ 4) ไม่ไอ
- ควรพิจารณาให้ยาต้านจุลชีพแก่ผป ู้ ่ วยโรคหัวใจรูห์มาติกและผูป ้ ่ วยภูมต
ิ า้ นทานโรคบก
(เช่น เม็ดเลือดขาวในเลือดต่า) ด้วย
โพรงจมูก (ไซนัส) อักเสบเฉี ยบพลัน (Acute Rhinosinusitis)
- มีอาการโรคหวัด หรือมีน้ามูกเรื้อรังหลังเป็ นหวัด อาจมีไข้ ปวดแก้ม หน้าผาก กรามบ
- X-Ray ไซนัสอาจพบเงาทึบ
- ส่วนมากเกิดจากไวรัส
- ควรใช้ยาต้านจุลชีพในกรณีทม ี่ ีสาเหตุมาจากเชื้อแบคทีเรีย
เกณฑ์วนิ ิจฉัยโรคไซนัสอักเสบเฉี ยบพลันจากแบคทีเรีย ได้แก่ขอ ้ ใดข้อหนึ่งดังนี้
1. มีอาการของหวัด ไซนัสอักเสบนานตัง้ แต่10 วัน โดยอาการไม่ดข ึ้
ี น
2. มีไข้สูง (39OC) ตัง้ แต่เริม ่ ป่ วย ร่วมกับน้ามูกเหลือง-
เขียวหรือเจ็บทีใ่ บหน้าต่อเนื่องนานอย่าง น้อย 3-4 วัน
3. มีอาการของหวัด ไซนัสอักเสบ นาน 5-6 วันแล้วอาการดีขน ึ้
แต่กลับมีอาการเลวลงโดยกลับมีไข้ปวดศีรษะ หรือมีน้ามูกมากขึน ้ เรียกว่า“ป่ วยรอบส
(double sickening หรือ double worsening)
