We, Windelino B. Coscos and Josephine S. Coscos, parents of Renz Darylle S. Coscos will not
able to process his NSO - Birth Certificate personally due to unavoidable circumstances.
Please accept this letter as confirmation that we authorize, our eldest son, Jowin Jeeh S.
Coscos, to transact any procedure to do on our behalf. Since, we are not in a position to come
to your office in ourselves; we have no objection for our son in signing documents if there is any.
We are willing to enclose any copy of our identification to prove that this request is valid.

For security purposes, we have attested below our own signature and
the signature of the authorized person.
