JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, GERRY BALUCA and NIÑO GONSALES, Filipinos, of legal ages

and residents of Trinidad, Bohol, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That we personally know CRESENCIA GALACIO-CAJA, likewise

a Filipino, of legal age, married to Juanito Caja and a resident of San
Isidro, Sierra Bullones, Bohol, Philippines, since she is our close family
friend;

2. That we know for a fact that said CRESENCIA GALACIO-CAJA

was born on April 19, 1964 at Sierra Bullones, Bohol to parents Eusebio
Galacio and Eustaquia Saludes;

4. That however, we have discovered that as reflected in the

Certificate of Baptism, her name was erroneously entered as
CRESCENCIO, and Marriage Contract as CRESCENCIA instead of
CRESENCIA ;

5. That we hereby declare and attest that the names CRESENCIA

GALACIO per Birth Certificate, CRESCENCIA GALACIO per Marriage
Contract and CRESCENCIO GALACIO per Certificate of Baptism refers
to one and the same persons living in the community;

6. That we are executing this affidavit to attest to the truth of the

foregoing statements and to establish the correct and real name of
CRESENCIA GALACIO-CAJA and for whatever other legal purpose this
may serve;
