The UA form will show at the top of the screen. Click the arrow in the
right corner of the form to move the form below the classroom.

Filling in the UA Form

The form is comprised of 3 parts: multiple choice, final score, and comments.

The teacher can fill in the form during or after class. The form can only be submitted
after the class has passed the 30 minute mark since the class start time. If the teacher
cannot finish scoring in one sitting, click “Save” to save progress and submit later.

Comment section requires between 50 to 1000 characters for submission.

UA Form Status
You can check the UA form status from the “Classrooms” tab of the teacher portal.

“Empty” – No form has been saved or submitted for this UA class.

“Saved” – A UA form has been filled in and saved. The UA form has not been submitted.
“Submitted” – The UA form has been filled out and submitted.

Additional Information
The UA form is not accessible before class.

After submission, UA form cannot be edited or submitted again.

Failure to submit or delayed submission can affect class

payment.
