Greetings!

I would like to inform your good office that I, JASTINE ROSE M. CAÑETE, a 4th year BS
Accounting Technology student of the university, is a Collegium Scholarship grantee for the
1st Semester SY: 2018-2019 and one of the grantees of Congressman Pantaleon Alvarez
through the STUFAPs CHED Scholarship Program.

Anent to this, I would like to ask for a refund for the both grants.. The said amount will be used
for a much important matter.

I am from Concepcion, Asuncion, Davao del Norte and I am currently an occupant of a

boarding house at Mabini Street, Tagum City. Together with my respect, I am anticipating on
your affirmative response.
