Accomplished Application Form and submit the following documents:

 For Incoming Grade 11 and SHS Graduate Students

 2 copies of latest 1x1 pictures (White Background with Nametag)
 Photocopy of Report card with at least 2nd Grading grade.
 For High School Graduate Students (Old Curriculum)
 2 copies of latest 1x1 pictures (White Background with Nametag)
 Photocopy of Report card

1  For Transferee and Second Courser

 2 copies of latest 1x1 pictures (White Background with Nametag)
 Present also any of the following:
 Photocopy of Transcript of Records (TOR)
 Passbook and School ID (For ZCSCP old Student Only)

Pay a non-refundable fee of P200.00 pesos at the

Cashier.

Secure a Testing slip at the Admission Office.

Proceed to the testing area on the scheduled date.

Claim the CAT results on 2nd week of April
2018.
