AFFIDAVIT OF CONSENT

Leyte State University (SLSU);

That I am giving my full consent to my said (son/daughter)to attend/participate in

the Practicum/Student Teaching on ________2018 at SLSU-CTE;

That I voluntarily waive any claim against the school/ supervisor-in-charge for
any untoward incident which may occur beyond control in the course of
his/her attendance/participation; after all precautionary measures and
exhaustive effort have been taken by the person-in-charge.;

That I am executing this affidavit to declare the truth of the foregoing facts as to
my undertaking to give consent to my (son/daughter)'s participation in the
Practicum/Student Teaching.
