The Department of Health is now accepting new applicants for its Doctors to the Barrios program!

Requirements for application are:

 Personal Data Sheet- 4 copies


 Statements of Assets and Liabilities Form (notarized)- 4 copies
 Diploma (Certified True Copy)- 1 copy
 Transcript of Records (Certified True Copy)- 1 copy
 PRC ID and Board Rating (authenticated)- 1 copy each
 NBI Clearance (original)- 1 copy
 Certificate of Specialty Board/Fellow/Medical Spec. Exam (if applicable)- 1 copy
Also include a cover letter indicating why you are interested in the program, to be addressed to:

The application period will run until the end of October. Requirements may be submitted to the
Health Human Resources Development Bureau (HHRDB) at the DOH Central Office in Sta. Cruz,
Manila or at any DOH Regional Office. Once you have submitted all your requirements, the HHRDB
