N479 Professional Activity Evaluation Form

Your Name: Rachel Eberle

Date of Activity: October 22, 2018

Faculty name: Linda Perez

1. Describe the community service/professional activity/s you attended or participated with (be

specific about the purpose and your role):

I attended a class to obtain my Advanced Cardiac Life Support (ACLS) certification. In this class

I reviewed concepts including: basic life support techniques, CPR, rescue breathing, AED use,

and cardiac rhythms. I also learned a lot of new information such as how to run a code blue/rapid

response in the hospital setting and what medications to give, and the order in which to give

them, in the event of a code blue/rapid response. I thoroughly enjoyed this class because it was

very hands-on which is how I learn best. We were given ample time to practice all the concepts

we had learned. We practiced CPR, rescue breathing, putting in an oral and nasal airway, and ran

a mock code. At the end of the class there was an exam to earn certification in advanced life

support.

2. Where was this held and what was the program/activity length?

This was an eight-hour class held at Eclipse CPR training facility on October 22nd, 2018. I

attended the entire class, as was required for certification, from 8:00am-4:00pm.

3. Discuss the value of this experience to you (be specific).

This experience was extremely valuable to my professional development, as I want to begin my

career on an Intensive Care Unit. I will likely experience many code blues in my career and

obtaining my ACLS certification was a really great way to learn and practice my skills before I
enter the work force. Practicing my skills and furthering my education will help me to remain

calm when I experience a real code blue.

4. Would you recommend this specific community/professional experience in the future; why

or why not? (Describe)

I would recommend this class to any nurse or nursing student. The class itself is very practical

and informative. I believe it is important to have advanced life support skills as a nurse,

especially as an ICU nurse. I would recommend Eclipse CPR Training Center because the

instructor was very engaging and made the learning environment fun. I felt very prepared for the

exam at the end of the class. There is also a large discount for students through this company.

5. Brochure or proof of attendance obtained? Yes, ACLS card with date of class.

Signature on validation of clinical hours form obtained? Yes.
