CCSHS SELECTION TEST S.Y. 2019-2020

GUIDELINES FOR ADMISSION
NATURE OF THE SELECTION TEST
The examination consists of a battery of Proficiency Tests in Science, Mathematics, English and
Logical Reasoning.
REQUIREMENTS TO BE SUBMITTED:
1. CCSHS Application Form
2. Photocopy of the school report card (SF9)
3. Photocopy of School ID
4. Two (2) pieces of 1 x 1 identical recent photographs

WHERE TO SECURE FORM

1.The Office of the CCSHS Principal/Guidance and Admissions/Registrar. At the back of the
Division Office, P. Sevilla St.,cor. 10th Ave. Caloocan City.
2.Offices of Elementary School Principals.

WHERE TO FILE THE FORMS

All forms duly signed and accomplished must be submitted to the Selection Test Committee c/o
Mr. Herwin Panganiban, School Clerk at the Principal’s Office, Caloocan City Science High
School between 9:00 a.m. to 4:00 p.m.

WHERE TO TAKE THE SELECTION TEST

The Selection Test will be administered on December 14, 2018, at Caloocan City Science High
School campus, P. Sevilla St.,cor.10th Avenue Caloocan City.
