INTRODUCTION AND PURPOSE

This Construction Safety Plan has been prepared to comply with the
regulations under Occupational Safety and Health Administration (OSHA), MNBC
and under The Guidance for the Management of Safety for Construction Works in
Japanese ODA Projects. It addresses foreseeable activities associated with the site
work activities to be conducted at the project: TAUNGOO-LEIKTHO-YARDO ROAD,
Construction Works for the Pilot Project in Bago Region for Regional Development
Project for Poverty Reduction, Phase II.

This plan is prepared to identify, evaluate, and control health and safety
hazards for the purpose of protecting all employees that involve in this project. The
plan provides for emergency response activities at the jobsite as well as covering
site hazard analysis, training requirements, engineering controls, materials handling,
and safe construction operations. The level of protection and the procedures
specified in this SAFETY PLAN represent the minimum health and safety
requirements to be observed by site personnel engaged in the referenced
inspection of construction related activities.

This plan is a site specific document. Technical, and/or Operational


Managers are responsible for ensuring all aspects of employee safety that are
addressed in this plan. Health and safety personnel are available to assist
management with the contents of the plan. The health and safety personnel help
ensure the plan complies with all regulations and policy described above.

This Safety Plan shall comprise of the following:

5. Safety Education and Training

7. Sharing Information

All Site Personnel Are Expected To Consider "Safety First" At All Times. The
ultimate responsibility for the health and safety of the individual employee rests with
the employee and his or her colleagues. Each employee is responsible for
exercising the utmost care and good judgment in protecting his or her own health
and safety and that of fellow employees. Should any employee observe a
potentially unsafe condition or situation, it is the responsibility of that employee to
immediately bring the observed condition to the attention of the appropriate
health and safety personnel as designated above and to follow-up the verbal
notification.

NEW WORLD SHWE PYI TAN has a strong commitment to providing a safe
and productive workplace. We seek to establish policies promoting high standards
of employee health and safety while delivering to our customer the highest quality
product. In keeping with this commitment we intend to maintain a positive Safety
Plan. Our employees conduct themselves and work in a safe manner with good
construction practices.

Effective safety demands cooperation on everyone’s part. It is important


communication is kept open at all times. For this reason, our management team
practices an open-door policy. Employees who notice hazards or other safety
problems or feel they need additional training must notify their supervisor.
Supervisors and management address these concerns and take corrective action
when warranted. Responsibility for achieving our safety goals belongs to the site
superintendent, supervisors, foreman, employees, and the safety officer with the
support of our management.

Everyone is obligated to know the safety requirements and standards for


their areas or job and abide by them. Supervisors must instill a positive attitude and
awareness of the “safety culture” in their workers through personal adherence,
training, personal contact, and regularly scheduled safety meetings. It’s the duty of
all employees to perform their work with maximum regard for their safety and co-
workers’ safety.

We shall appoint the personnel, Project Manager, Health and Safety Coordinator
(HSC), Site Safety Officer (SSO) and Site Safety Supervisors (SSS) for internal
organizational structure for safety management for this project. Key personnel and
their responsibilities are described below.

Project Manager: Provides review of all site activities and directs site activities via
the Site Safety Officer reports to Client. Has authority to direct response operations.

Responsibilities:

■ Prepares and organizes the background review of site conditions, the site
safety and the field team.

■ Obtains permission for site access and coordinates activities with


appropriate officials.

■ Briefs the field team on their specific assignments.

■ Coordinates with the Site Safety Officer (SSO) to ensure that health and
safety requirements are met.

■ Prepares the final report and support files on the response activities.

■ Serves as the liaison with public officials.

Health and Safety Coordinator (HSC)

Responsibilities:

■ Assists SSO with development of the CHASP, updating CHASP as dictated


by changing conditions, jobsite inspection results, etc.

■ Assists SSO in conducting Jobsite Safety Inspections and assists with the
correction of shortcomings found.

■ Ensures training requirements are satisfied in a timely manner.

■ Maintains all records on personnel (medical evaluation results, training


and certifications, accident investigation results, etc.).

■ Prepare any Root Cause Investigation Reports/Preventative Action Plans


for any incidents and or Close Calls.

Site Safety Officer (SSO): Advises the Project Manager and HSC on all aspects of
health and safety on site. Stops work if any operation threatens worker or public
health or safety.

Responsibilities:

■ Manages field operations.

■ Enforces safety procedures.

■ Coordinates with the SSO in determining protection level.

■ Enforces site control.

■ Documents field activities and sample collection.

■ Serves as a liaison with public officials.

■ Ensures that all necessary Health and Safety equipment is available on


site and is functional.

■ Periodically inspects protective clothing and equipment.

■ Ensures that protective clothing and equipment are properly stored and
maintained.

■ Controls entry and exit at the Access Control points.

■ Coordinates health and safety program activities with the HSC.

■ Confirms each team member's suitability for work based on a physician's


recommendation.

■ Monitors the work parties for signs of stress, such as cold exposure, heat
stress, and fatigue.

■ Implements all elements of this Safety Plan.

■ Conducts periodic inspections to determine if this Safety Plan is being


followed.

■ Knows emergency procedures, evacuation routes, and the telephone


numbers of the ambulance, local hospital, poison control center, fire
department, and police department.

■ Notifies, when necessary, local public emergency officials.

■ Coordinates emergency medical care.

■ Sets up decontamination lines and the decontamination solutions


appropriate for the type of chemical contamination on the site.

■ Controls the decontamination of all equipment, personnel, and samples


from the contaminated areas.

■ Assures proper disposal of contaminated clothing and materials.

■ Ensures that all required equipment is available.

■ Advises medical personnel of potential exposures and consequences.

■ Notifies emergency response personnel by telephone or radio in the


event of an emergency.

■ Assist in the preparation of all Root Cause Investigation Reports/


Preventative Action Plans for any incidents and or Close Calls.

3. Promotion of the PDCA Cycle

NEW WORLD SHWE PYI TAN shall set out the basic principles for promotion of
the PDCA Cycle at construction sites in accordance with Clause 2.3 "PDCA for
Safety Management" which is described below.

The basic principle of PDCA for safety management shall be the cycle of
"Plan, Do, Check, Act" with "Plan" being the process of establishing the Safety Plan
and its Method Statements on Safety, "Do" being the specific implementation of
the plan thus established, "Check" being the observation and confirmation of the
safety management process, and "Act" being the implementation of
improvements to the implemented plans based on the past performance to ensure
the continuous development of field site safety standards. The cycle of these
processes shall be defined as PDCA for safety management. NEW WORLD SHWE
PYI TAN shall have chief responsibility for the implementation of safety
management.

Formulation of the "Plan": As part of the "Plan" NEW WORLD SHWE PYI TAN shall
formulate the Safety Plan as the basic safety management. NEW WORLD SHWE PYI
TAN shall then formulate a Method Statements on Safety as the more detailed
safety management plan for implementation.

Broad Publication at the "Planning" Stage: NEW WORLD SHWE PYI TAN shall
document the items necessary to ensure safety at construction sites, disclose the
same to all Project Stakeholders and ensure that those items are fully understood at
the "Planning" stage.

"Do": NEW WORLD SHWE PYI TAN shall implement safety management on
construction sites according to the Safety Plan and its Method Statements on
Safety as formulated at the "Planning" stage.

Observation and confirmation ("Check"): The Employer/Engineer shall review the


NEW WORLD SHWE PYI TAN’s implementation of the "Doing" stage in accordance
with the Safety Plan and the Method Statements on Safety and give guidance
where it is in any way insufficient or incorrect.
We shall check our own implementation of the "Doing" stage and
implement improvements where insufficient in any way. The managing office's
safety managers shall make regular observations and undertake checks.

Disclosure of results at the "Check" Stage: NEW WORLD SHWE PYI TAN shall
document and disclose the results of the "Check" stage to the Project Stakeholders.

“Act”: NEW WORLD SHWE PYI TAN shall examine specific methods of implementing
safety measures or related management systems and take corrective action
based on their results at the "Checking" stage. In addition, we shall review our
Safety Plan and the Method Statements on Safety and submit revised versions to
the Employer/Engineer. The Employer/Engineer shall review the submitted
documents.

Broad publication of the results of the "Acting" stage: In case the Safety Plan or the
Method Statements on Safety is revised, NEW WORLD SHWE PYI TAN shall document
the revision and disclose such revision to the Project Stakeholders. In particular, the
Contractor shall explain the types of work to which such change is to be
implemented and ensure that, prior to the commencement of the relevant work,
the change is understood by workers who engage in such work.

Investigation of causes of occupational accidents: If an occupational accident


takes place, the Employer, Engineer and NEW WORLD SHWE PYI TAN shall suspend
construction work to the extent necessary during the necessary period and
investigate the cause. NEW WORLD SHWE PYI TAN shall remove the cause in
accordance with the basic principles of safety management, clarify measures to
prevent the risk of accidents occurring and resume work with the approval of the
Employer. NEW WORLD SHWE PYI TAN shall re-examine the Safety Plan and the
Method Statements on Safety based on the results of the examination and revise
them as required. The Employer/Engineer shall review those documents if revised.

Continuous improvement: NEW WORLD SHWE PYI TAN shall ensure that the PDCA
safety management process is implemented and continuously improved in order
to maintain safety at construction sites.

4. Monitoring

NEW WORLD SHWE PYI TAN shall set out the basic principles for monitoring
safety management while considering the following requirements:

(1) Monitoring by NEW WORLD SHWE PYI TAN

NEW WORLD SHWE PYI TAN shall undertake tests at all areas on site to
determine the extent of implementation of and compliance with safety
management principles in accordance with the Safety Plan. In addition,
NEW WORLD SHWE PYI TAN shall undertake tests to determine the extent of
implementation of strategies for the safe execution of each type of work in
accordance with the Method Statements on Safety.

(2) Monitoring of accidents or injuries

NEW WORLD SHWE PYI TAN shall report to the Employer/Engineer in


accordance with the contract documents in the event of injury attributable
to an occupational accident or construction work. On receipt of such
report, the Employer/Engineer shall notify the competent governmental
organization and JICA of the accident or injury in accordance with the
relevant laws and regulations of the Republic of the Union of Myanmar and
the contract documents. The Employer, Engineer and NEW WORLD SHWE PYI
TAN shall keep records of all such reports and maintain them until the
completion of the work.

(3) Monitoring near misses


NEW WORLD SHWE PYI TAN shall collect and analyze information on
dangerous incidents that do not result in occupational accidents but may
lead to such accidents ("near-misses") and utilize that information to prevent
future occupational accidents. The Employer/Engineer shall monitor the
NEW WORLD SHWE PYI TAN’s implementation of safety management
principles and give appropriate guidance to the Contractor where they find
any safety-related problems.

NEW WORLD SHWE PYI TAN shall set out the basic principles for education
and training on safety to maintain safety during the construction works and take
into account the following requirements:

(1) Compliance with laws and regulations of the Republic of the Union of
Myanmar on education and training on safety

(2) Education to all Project Stakeholders (and to all new entrants to the site)
on:

1) An overview of the construction site


2) General rules on the construction site including the Safety Plan
3) Protective gear
4) Items necessary to ensure safety when carrying out work in addition to
1) through 3) above

(3) Education on the Method Statements on Safety for the assigned work

(4) Education when changes are made to work

(5) Education and training for special workers

1) Workers engaged in work that is regulated by the laws and


regulations of the recipient country
2) Operators or drivers of construction machinery or equipment
3) Workers engaged in work in excavated areas, shafts, underground
passages or tunnels
4) Workers handling explosives and engaged in blasting work
5) Workers engaged in work that uses compressed air
6) Workers engaged in reinforcing bar work, concreting work, or
formwork
7) Workers engaged in other kinds of work belonging to special
categories

(6) Education and training for emergency response personnel

(7) Education for visitors, Education for third parties (other than Project
Stakeholders) when they enter the work area

(8) Training for emergencies and unforeseen circumstances

(9) Activities to promote safety awareness

6. Voluntary Safety Management Activities

NEW WORLD SHWE PYI TAN shall set out the basic principles for voluntary
safety management activities while taking into account the provisions in the tender
documents, the contract documents and the following requirements:

(1) Morning meetings on safety

(2) Foreseeing hazardous activities

(3) Tool box meetings

(4) Safety rota systems

(5) Regular, monthly and periodic inspections

(6) Sorting, decluttering and cleaning

(7) Safety conventions

(8) Safety patrol

(9) Near-miss reporting system

(10) Other activities

NEW WORLD SHWE PYI TAN shall set out the basic principles for sharing
information necessary to ensure effective safety management while taking into
account the following requirements:

(1) Description of education for new entrants

(2) Other information necessary to maintain safety

8. Responding to unforeseen circumstances

NEW WORLD SHWE PYI TAN shall determine the policy for responding to any
unforeseen circumstances considered to be caused by natural disasters such as
rainstorms or earthquakes while taking into account the following requirements:

(1) Emergency evacuation procedures

(2) The establishment of an emergency communication network system

(3) Procedures for responding to unforeseen circumstances

(4) Collection of weather information
