3. Log sheets/book
In order to qualify for re-certification, you will need to demonstrate that you have carried out satisfactory work
activity with reasonable continuity during the previous five years. “Reasonable continuity” means that an absence of
change or activity (preventing you from practising the duties corresponding to your certificate) for one or several
periods during the validity of the certificate does not exceed a total of one year. Certificate holders not able to
satisfy the continuity rules will be treated as initial candidates.

EMPLOYMENT LOG SHEET –(Please complete a log sheet for each employment)
Carry out material incoming receipt inspection and verification with relevant documents or
certificates from supplier or sub-contractor /vendor.
Arrangements of all the fabrication activities like fit up, welding, NDT tests, Machining & Sub
assembly in the sequential order as per the work instructions given by the clients.
In Fit-up Assembly Water Level, Theodolite checking and Level Meter Checking, Bolt Tight
Checking with Feeler Gauge
Verify welding machines, oven condition and calibration. Monitor welding is done as per WPS
Report Non-conformance with reference to applicable acceptance criteria, to carry out corrective
action and closing NCR’s with required back up documents if required
Continuous monitoring & leading the site supervisors & workers for the optimum utilization of all
the resources as well as the completion of the job in time.
Preparation of Inspection Reports such as
Raw materials inspection reports
Fit-up & stage inspection reports
Weld summary & weld visual inspection reports
Internal parts assembly reports
Final Inspection reports
Coordination with the clients regarding the job progress, drawing Clarifications and for any day to
day problems.
Witness for welder Qualification test.
Witness for Material Inspection and review of Material Test Certificates.
Witness for WQT Mechanical tests (Tensile strength, Root bend and Nick break test).
Witness for Pressure vessel Final Dimensional Inspection.

Witness for Pneumatic test (leak test).
Monitoring the all fabrication activities to relating job.
Review of Welder Qualification Test records.
Review of drawings, WPS, use of consumables, NDT test procedures and methods, Mechanical
Clearance for Hydro-test and Punch list clearance.
Review of Presser vessel PWHT reports.
Review the all NDT reports and mechanical test reports.
Review the final Documents.
Witness for Pressure test and check the Instruments conditions and calibration test certificates.
And all Daily activities reported to my superior.
Familiarize with Approved Governing Standards and Specifications
Familiarize with the Approved Quality and Inspection plan.
Supervision of sub-contractor painting Inspectors.
Witness/Review Weather Conditions, salt contamination test , blast profile,
WFT/DFT checks, holiday test, pull of test etc.
Verifying project Equipment, Consumables and Material inspection.
Co-ordinate with Project Management Company for QC activities.
Planning and controlling quality activities.
Coordinating with Quality Inspectors.
Supervising the Coating/Painting Repair Activities.
Prepare Daily Inspection Reports and final documentation.

CSWIP Inspectors are required to keep themselves up-to-date with technical developments in their field within the
Industry. Continuing Professional Development is required for your CSWIP Re-Certification. 35 hours of CPD per year
is recommended.
Professional development may be achieved in any of the following ways, depending on your personal circumstances,
learning style and opportunities available to you.
Private Study: Such as distance learning, Open University, Writing papers, Reading, Research on the
internet/journals etc.
Short Courses: Attendance at short courses, seminars and presentations
Attending Conferences, symposia and exhibitions
Additional Study: Learning foreign languages, new computer skills etc.
WJS/Professional Membership
Meeting (non-social): Attendance at branch or technical meetings/webinars
Further Education Studies
Imparting knowledge: Making presentations, preparation of papers accepted for conferences/publications,
coaching/teaching/lecturing

To comply with certification requirements, all CSWIP & BGAS-CSWIP Certificate holders are
required to submit results of an eye test taken within the last 2 years.
ALL CSWIP & BGAS-CSWIP CERTIFICATE HOLDERS MUST TAKE A NEAR VISION TEST:
Is the above named person capable of reading Times Roman N4.5 or Jaeger 1 on a standard reading
test plate at a distance of no less than 30cm unaided or corrected in at least one eye?
All BGAS-CSWIP & CSWIP NDT Certificate holders must also provide the results of a colour
perception test.
Colour perception shall be assessed by the Ishihara 24 plate test.
Radiographic Interpretation, Radiographic Inspector, Senior WI 3.2.2 & TOFD certificate holders
are also required to complete a Shades of Grey Perception Test.
Eyesight tests provided by opticians/hospitals etc. will be accepted as long as they clearly state that all of the
requirements have been met. If needed or for guidance please use this TWI CL Eye Test Form. Any observed
difficulty during the eye test should be reported to the employer.
