Please accept this letter as a formal application for my leave of absence from December to January
2018. Due to my brother’s condition, I need to go home to our province, Capiz—he was diagnosed
with pineal tumor (brain tumor) that need immediate Tomotheraphy/ Chemotherapy. Our both
parents were already died,thus, I would like to knock in your generous office to grant my request to
allow me to get the leave and accompany my brother during this stage of his life.

My other siblings, Eunice the second to the oldest, already availed her leave from her work as a public
teacher during my brother’s hospitalization and brain operation had already absences that will be
deducted to her salary. Furthermore, Mikko second to our youngest, cannot renew her contract this
December as a clerk in Union Bank if she will continue to absent again for the long time to replace my
other sister on taking care of our brother.

I will return to work on________________
