SURVEY QUESTIONNAIRE

This study is being conducted by Zamboanga City State Polytechnic College students as
part of our requirements in our course Civil Engineering Project I. This is a questionnaire that
survey about the location, community, and existing strucuture status affected by the Typhoon
Pablo and Vinta from the past few years.

The purpose of this questionnaire is to gather information that gave clarification and
insights to the proponents that will be apply for the study entitled “Proposed Seawall along
Lower Calarian Shoreline”. The questionnaire is randomly distributed among Lower Calarian
citizen.

In this section we will ask concerning people based on the following status that mentioned
above. These are the following questions:

A. What is the status of Lower Calarian shoreline area?

B. What is the status of the existing revetment at Lower Calarian

C. What is the status of the location in terms of hydraulic condition?

1. Height of extreme high tides? Approximate?

D. How many families affected by the storm surge caused by the Typhoon Vinta?
Approximate?

E. Length of shoreline at Lower Calarian ?
