CHAPTER 1
GENERAL PROVISIONS
1. For the Filipino people who regard the Armed Forces of the Philippines as
the defender and shield of the nation, unprodded performance and excellent
service is called and expected. Such privilege has always been safeguarded
by the AFP workforce and deemed an honor worthy to become a part of. This
mindset is evident in the numerous men and women entering the military
service given the nature and demands of the job, apart from the quality of
work they put across in defending the Constitution and the Republic of the
Philippines.
1. Background
a. The acts of awarding and honoring have long been practiced, adopted
and have continuously weaved with the evolving systems, organizations and
settings throughout history. This can only attest to their role and value which
remain imperative and enduring.
1) Core Characteristics
a) Fair – Fairness is being free from bias or treating all sides alike,
justly, equitably, and in conformity with existing rules and standards. The AFP
Awards and Decorations System adheres to a common standard and
guidelines and ensures that recipients are worthy and have demonstrated
exceptional heroism, achievement or service based on the criteria of the
awards. The process in itself starts with an apolitical and biased-free
recommendation by the Commanding Officer of a unit or a competent
personality who has knowledge of the deed or act, achievement or service.

2. Classification of Awards
a. Decorations – These are medals and badges awarded to individuals to
recognize their exceptional accomplishments in line or outside their call of
duty, or their outstanding services to the AFP or the Nation as a whole. The
following are the authorized decorations of the AFP in their order of
precedence where the primacy follows; the ranking of the approving authority,
and combat-related heroism/achievement over other types of awards.

CHAPTER 2
AWARDS AND DECORATIONS PROCEDURES AND
THE DIFFERENT BOARD COMPOSITIONS
The AFP, as a general policy, publicly and timely recognizes the acts of
heroism, exceptional services and achievements of every AFP personnel.
This chapter provides the process guidelines and policies governing the AFP
Awards and Decorations to ensure the uniform and standardized
implementation in all AFP levels of command.

a) Deliberation of awards shall take two (2) weeks utmost from the
receipt of the recommendation.

(1) One award shall be made for the same act of heroism,
extraordinary achievement, or period of meritorious achievement.
(2) Duplicating awards shall not be made for outstanding
achievement and inclusive meritorious service within the same period of time.
Continuation of the same or similar type of duty previously recognized by an
award shall not be the basis for a second decoration.
(3) If appropriate, an award maybe made to incorporate the
extended period of services by superseding or amending the earlier award.
(Illustration: MMM as service award issued to personnel who is presently in
his position for more than three years. Upon his relief, that issued MMM shall
be amended to include the extended service).
(4) For the service awards (i.e, DSS, MMM-service), relief from
position is a mandatory requirement. Except for MMM in which case the
award can be bestowed to one who is still in his duty assignment provided he
has served in said assignment for a continuous minimum period of three
years.
(5) In the determination of whether or not relief is required for
MMM award, distinction should be made, thus the MMM award for
achievement or heroism does not require relief from duty or position.
d) The Board shall scrutinize the submitted reports and validate the
attachments by the unit commanders or through conduct of interview on the
involved personnel when possible.

(4) In each case, the fact, date and place of presentation shall
be reported to the adjutant of the unit concerned.

b) The unit/office that issued the order for the award, upon re-
validation of the given cause/s, cancels the award. The action taken shall then
be recorded prior furnishing a copy to the directing/requesting unit.

2) For all higher awards, the AFP Adjutant General, Deputy Chief of
Staff for Personnel, J1, the AFP Deputy Chief of Staff, in that order, review the
AFP Awards and Decorations Board’s recommendations and communicate
such recommendations to the CSAFP for his approval and or endorsement
and where the CSAFP approves awards within his authority.

Board may require the presence of witnesses and resource persons, including
the MFV recipients, to testify during the deliberations.
d) It must involve the risk of life. A complete and utter disregard for
the personal safety of the recommendee must be proven. It must be
understood that no one among the personnel of the unit engaged in the battle
would have normally done the singular act of valor for which the
recommendee has been cited. Moreover, the recommendee acted on his own
volition or initiative and that no one gave him any specific orders to do the
acts of bravery that he did.
g) It must also be made clear that even if the act of supreme valor
was not made, the personnel in the unit or at least the recommendee have
done all that the honor of war requires such that whatever happens
afterwards, even if the members of the unit surrendered or retreated without
orders, the overwhelming odds against them has made such action
acceptable within the parameters of honorable combat.

2) For the MFV Award, the MFV Board further transmits its own
recommendation to the Board of Generals (BOG) after deliberation. The BOG
then reviews the MFV Board’s recommendations. The purpose is to present
before the members of the BOG the recommendation of the valor award in
order to obtain their approval as to the nobility and dignity of the deeds and
confirm the MFV Board’s recommendation.
4) The BOG Secretary then transmits the Board’s actions to the DCS
for Personnel, J1 for the preparation and packaging of the recommendation
for endorsement to the President through SND. The (BOG) shall provide the
approved minutes of the BOG meeting to the DCS for Personnel, J1 as part of
the documents in the processing of MFV recommendation to the CSAFP
through channels.
5) The CSAFP (who also sits as the Chairman of the BOG) then
approves the recommendation of endorsement of the award to the President
through the SND. The SND shall establish within the Defense Department
their own procedures and requirement in the processing of the MFV Award
which shall be in accordance with the criteria and parameters of the award set
forth in this manual.

a. The Medal for Valor (MFV) Board: The MFV Board shall deliberate
recommendations of MFV awards as endorsed by the AFP Awards and
Decorations Board to ensure propriety, fitness and attain the well-meaning
intent of the Valor award. The Board in the execution of its responsibility, shall
preserve the true significance and highlight the value of the MFV award.

The Secretary, MFV Board: The MFV Board Secretary shall undertake
the initial screening and processing of recommendations and shall schedule
board deliberations thereof. Specifically, the secretary ensures the
completeness of the documents and appropriateness of the recommendation,
prepares the minutes of board deliberations and resolutions for the Board
members’ approval, collate all pertinent documents and upon
recommendation of the Chairman, MFV Board, transmits the recommendation
to the AFP Board of Generals for final deliberation through the Deputy Chief
of Staff for Personnel, J1.
b. The AFP Awards and Decorations Board: The AFP Awards and
Decorations Board shall deliberate recommendations for higher awards,
usually those awards that need the approval of the Chief of Staff, AFP
(CSAFP) Secretary of National Defense (SND), or the President.
c. The GHQ Awards and Decorations Board: The GHQ Awards and
Decorations Board shall deliberate recommendations of awards coming from
GHQ Offices, AFP Wide Support and Separate Units, and Unified
Commands. The GHQ Awards and Decorations Board shall transmit its
recommendations through channels to the Commander, GHQ and HSC who
approves awards within his authority and endorses higher awards that need
the approval of the CSAFP, SND, or the President to the CSAFP through the
DCS for Personnel, J1.

f. Deputy Chief of Staff for Personnel, J1: The Deputy Chief of Staff for
Personnel, J1 shall endorse recommendations of all deliberated awards to
approving authorities while ensuring compliance to recommendation
procedures. J1 shall direct issuance of order of all approved awards to the
Adjutant General.

OTHER GUIDELINES AND PROVISIONS
1. Limitations
a. Only one award shall be made for the same act of heroism,
extraordinary achievement, or period of meritorious service. It shall mean that
the cited act can only be recommended once (individual or group), thus, the
same act cannot be used as a basis in another recommendation. Such is
tantamount to justifying the grounds for revocation as specified in the previous
paragraph (2-1, f) of this manual.
a. Bronze Anahaw Leaf – Shall be awarded for the first, second, third and
fourth succeeding deeds instead of the Medal for Valor, Philippine Legion of
Honor, Outstanding Achievement Medal,Distinguished Conduct Star,
Distinguished Service Star, GawadsaKapayapaan, Gold Cross, Distinguished
Navy Cross, Distinguished Aviation Cross, Silver Cross, Meritorious
Achievement Medal, Distinguished Service Medal, Chief of Staff
Commendation Medal and Ribbon, GawadsaKaunlaran, Bronze Cross, Silver
Wing Medal, Military Merit Medal(Service),SagisagngUlirangKawal, Military
Civic Action Medal, Armed Forces Conduct Medal, PA, PN(M), PAF & AFP
Command Reservist (Volunteer) Officer and (Volunteer) Enlisted Personnel of
the Year Medal and Ribbon, Kagitingansa Barangay.

a. In the case of units of the Armed Forces of the Philippines, which may
be a recipient of a foreign award for invaluable services rendered, the
Commander of such unit shall seek authority from the Chief of Staff, AFP to
receive such award and shall become the authority to award it to the
individual members thereof.

completeness and appropriateness of the submitted documents, schedules
for board deliberation, prepares minutes and board resolution and
subsequently endorses the same to the AFP Regulations Board for further
deliberation.

10. Sanctions. The disciplinary powers of the commanding officers under the
present Article of War 105 shall apply for any transgressions committed by
members of the Armed Forces in lieu of the implementation of the provisions
stated herein.

e. When decorations and service medals are worn, under the provisions
of para, b and c above, all those to which the wearer is entitled shall be worn
by the persons in formation; persons not in formation, including the reviewing
and inspecting officers and their staff, may omit the wearing of the same.
2. How Worn
b. Badges – May be worn at all times when in uniform. These badges are
worn above the line of the medals or ribbons on the left breast pocket and on
the right breast packet, respectively. Moreover, there shall be no limit in the
number of badges to be worn given that they are worn evenly on the left and

right side and appear appropriate on the uniform. Likewise, the badges worn
shall not impede movements.
1) Service ribbons may be worn on the coat, or blouse. They shall not
be worn on mess jacket, combat uniform or civilian clothes overcoat.

a. Obverse
a) Disc – It is in gold with a golden sea lion and water waves on its face.
It is resting on a cross and crossed-swords.
b) Sea lion – It is a relief in gold holding the Philippine sun and placed
symmetrically on the disc. Portions of the body and the tail are submerged in
water. The tail end is visible.
e) Crossed swords – They are in gold, placed in level with the cross and
also covered by the disc. The blades are at dexter and sinister chiefs, and the hilts
are at a dexter and sinister base. Both tangs are tanged to the sides of the cross at
dexter and sinister bases.
f) Bar – It is in gold. The top and lower sides have grooved borders.

serving as Automatic Rifleman of the Command Group, 3rd Scout Ranger Company, 1st Scout
Ranger Battalion, First Scout Ranger Regiment, Special Operations Command, Philippine Army
during as encounter against more or less 100 armed elements of the Moro National Liberation Front
- Misuari Faction at vicinity Targets and 24, Barangay Sta Barbara, Zamboanga City on 241400H
September 2013. Despite being wounded and hospitalized brought about by his previous encounter
with the RMEs o 13 September 2013, Private First Class Paquit voulunteered to return to ground
zero to continue the fight for a one antion and one flag and bring Zamboanga City to nomalcy. His
display of intrepidity and courage during the conduct of a coordinated attack on Targets 5 and 12 in
order to neutralize enemy forces has weakened the enemy from thier location. Subsequently, heavy
exchange of fires erupted between the enemy and trapping section of 3SRC at Target 3. Due to the
massive firepower coming from the well-entrenched enemy forces and deteriorating cover, Private
First Class Paquit repositioned his squad automatic weapon in order to deliver accurate fires toward
enemy strongholds, Aminds impending danger and deep concern to the repositioning troops, Private
First Class Paquit adjusted his position to a better line of sight but more vulnerable to enemy fire, not
minding the danger that may come and continued to were able to reposition with only minimal,
resistance felt from the enemy. With his courage, dedication and unselfish attitude, further casualties
were avoide dand the neutratlization of enemy firing positions greatly contributed to the clearing and
capture of enemy strongholds on Targets 5 and 24. A a result of his selfless act, a bullte pierced his
left neck that caused his fatal dath. The operation resulted to the capture of 41 MNLF fighters, 15
enemy kille (BC), recovery of 41 assorted firearms, 44 assorted magazines, 796 rounds of
ammunitions, two (2) pcs Camera, one (1) Iphone, one (1) cellular phone and other war materiel
with high interllience value. By this display of heroism, Private First Class Paquit distinguished
himself in combat, in keeping with the finest tradition fo Filippino soldiery.

along with the Allied Liberation Forces symbolizing the triumphant return of the
Philippine Government from exile in the United States. Immediately, he re-
established the national government in Tacloban, Leyte, where he exercised moral
leadership and persuasion over all guerrilla troops in their operations against the
enemy forces. He worked together with the American liberators n reconstructing a
war ravaged nation. On 23 October 1944, he accepted the power and functions of
government from General Douglas MacArthur with the herculean task of putting civil
affairs in order. Highly nationalistic, he led the Filipino people in gaining sovereign
status in moving forward to independence and economic development. By these
achievements, PRESIDENT OSMEÑA earned the undying respect and gratitude of

LIEUTENANT GENERAL YAP steered the Philippine Army in counterinsurgency operations.
Under his leadership, Army units neutralized 3,138 enemy personalities, recovered 1,095
firearms, cleared 532 affected barangays and caused the return to the folds of the law of
23,056 communist terrorists. Aware of the need for readily mobilizable reservists, he caused
the training and enlistment in the reserve force of 58,672 reservists; made possible the
training of 38 reserve companies; and organized 12 ready reserve battalions. He initiated
reforms in personnel management, emphasizing on discipline , morale and welfare of Army
personnel, weeding out scalawags and misfits, and imposing punishments on other erring
personnel. He prescribed procedures and policies that ensure the promotion and the
designation to key commands and staff positions of the best qualified officers. He pursued a
year-round physical fitness and athletic program, enabling the Army to become the overall
champion in the 1994 AFP-PNP Olympics. On civil-military operations, the Army undertook
medical-dental civic action, community relations and literacy patrols. He vigorously
institutionalized the Army’s livelihood programs nationwide, benefiting Army personnel and
their dependents, particularly those separated for physical disabilities. Under his supervision,
Army engineers met timetables in their infrastructure projects, like the construction of PFVR
buildings, roads, bridges and the launching of ecology-enhancing activities. He initiated
visioning exercises to formulate the framework of the Philippine Army of the 21st century and
its 10-year modernization plan, the Army’s preparation for the Fort Bonifacio conversion plan
and the Army Base Force in the year 2000. He linked closely with the BCDA and succeeded
in getting commitments for the relocation in socialized housing units of about 7,000 families
composed of soldiers, retirees and civilians. By these achievements, LIEUTENANT
GENERAL YAP earned the respect and admiration of the Philippine government and the
Filipino people.

Infantry Division, Philippine Army. Tasked to isolate and subsequently destroy members of
the MNLF Misuari Faction who took around 200 civilian hostages and plundred seven (7)
coastal barangays of Zamboanga City, LIEUTENANT COLONEL REY P BARROQUILLO
JUNIOR led his battalion in 27 major encounters to seize objectives in Barangay Sta
Caltalina and Sta Barbara, all of Zamboanga City. Despite heavy enemy resistance built
around concrete walls and buildings and the presence of hostages, he personnally and
gallangry led the rapid forward movement of his battalion thereby trapping hundreds of
enemies holed out in Barangays Sta Catalina and Sta Barbara. Unmindful of his safety
from enemy sniper and mortar fires, he ran from one subordinate unit to another to rally his
troops and conduct detailed leader’s reconnaissance to enhance his situational awareness
and facilitiate more accurate planning and decision making. After setting the trap, he led
his battalion in anohter rapid movement to destroy trapped enemy forces in Martha and
Ayer Villages and rescue the hostages. Through his frontline leadership, which translated
to effective planning, synchorinzation of firepower and maneuver and maintaining the
initiative, the members of the MNLF-Misuari Faction holed outh in his battalion’s sector
were literally wiped-pit. After 21 days of heavy and continuous fihgting, his battalion
overran all enemy positions in sector. His battalion accounterd for 42 enemy body counts,
37 enemy captured, 43 high powerd firearms recoverd and ight (80 hostages rescued. By
these achievements, LIEUTENANT COLONEL REY P BARROQUILLO JUNIOR

their fires, be making sure that each approaches or exits of the enemy were
manned and continuously being monitored and fired upon. Realizing the adversity
of situation the rebels have gone into, he audaciously closes in into the exit holes
where the troops expected that rebels will come out as they were been captured.
As a result, the unit was able to capture 58 Rouge MNLF rebels, the rescue of four
(4) hostages and the recovery of 22 High Powered Firearms. By these

surrender of other rebels to other units who were in close proximity of the exits where the

joined the wounded from the previous Gamba deployed towards Target 12. With
limited and deterioration cover from tremendous enemy direct and indirect fires
coming from Targets 12 and 6, the assaulting courage and determination to seize
the final target and after breaching a hole on the concrete wall of Target 12,
Corporal Binauhan slung on his rifle, took two (2) hand under heavy enemy fires
from the north and west direction. Upon closing in, he quickly rolled the grenades
inside the said objective instantly killing four (4) enemies. As a result, the
assaulting team was able to move forward and further seized the target. The series
of encounter resulted to four (4) enemies killed (Body Count) and the recovery of
five (5) high powered firearms, one (1) jungle pack, four (4) assorted magazines,
five (5) bandoleers and one (1) trigger mechanism for M14. By this gallantry,
Corporal Binauhan distinguished himself in combat which is in keeping the finest
tradition of Filipino soldiery.

September 1997. These officers actively participated, without regard to their personal
security and safety, in the conduct of coordinative dialogues/meetings with MILF

camps/strongholds. The conduct of dialogues/meetings created a peaceful and
conducive atmosphere for the resumption of exploratory talks between the GRP and
the MILF Peace Panel. Through back channeling efforts aimed at ending the
hostilities and resumption of peace talks which was temporarily stalled by outbreak of
the all-out war during the Estrada Administration, they were instrumental in the
committee’s relentless efforts in preventing and/or de-escalating of hostilities. They
pursued all diplomatic means to stop the hostilities between the BIAF-MILF and
government forces. Their action precluded further damage to properties as well as
injuries and/or death to both innocent civilians and government forces. By these
achievements, these officers earned lasting honor not only for themselves but also
for the Armed Forces of the Philippines.

Kalungiking, Poquin Corner, Kalinga-Apayao on 29 August 1994, while serving as
Company Commander of Bravo Company, 17th Infantry Battalion, 5th Infantry Division,
Philippine Army. Tasked to locate and recover the arm cache of the Northern Luzon
Operational Command of the New People’s Army, SECOND LIEUTENANT FLORES led
his platoon to its objective. After deactivating land mines and avoiding booby traps laid
along the route, he led his men along with other platoons, in creeping towards the enemy
position. As the troops opened fire, the terrorists behind fortified barricades retaliated with
heavy volume of fire. He leapfrogged to close in on the enemy and fought fiercely the
enemy repelled their counterattack while providing cover fires to allow the other platoons to
maneuver. After 30 minutes of firefight, the troops seized the enemy positions, forcing the
latter to withdraw, leaving six dead comrades. On the scene of encounter, the troops
recovered ten high-powered firearms and later found the arm cache. By this gallantry,
SECOND LIEUTENANT FLORES exemplified the finest tradition of Filipino soldiery.

Unmindful of his own personal safety he courageously led his squad in defending their
blocking position against fiercely attacking enemies who intends to breakthrough their
defensive line and escape. When his squad’s machine guns were down, the enemies
were able to get close to his gunners but he fiercely grabbed one (1) attacker by
engaging close hand to hand in combat against the enemy trying to outflank his troops
until he was able to recover the enemy’s rifle and fired it against the attackers. With his
utmost display of courage despite adversities, SGT DUQUE have uplifted the moral of
his troops and showed an example of a soldier’s determination to fight. Despite their
losses, his squad repelled the enemy’s attack. The fierce firefight lasted for twenty (20)
minutes that resulted to two (2) enemy killed (BC) with undetermined number of
wounded and recovery of one (1) AK47 and two (2) M16 rifles. By this gallantry, SGT
DUQUE exemplified the finest tradition of Filipino soldiery.

and 225, 206th Tactical Helicopter Squadron, 205th Tactical Helicopter Wing, Philippine Air
Force during the conduct of aerial reconnaissance and strafing mission over the conflicted
ares of Zamboanga City on 23 September 2013. Undaunted by imminent threat of hostile
enemy presence, these pilots and crew performing strafing mission at the spotted boats,
suspected used as transport vehicles of the Moro National Liberation Front – Misuari
Faction, hiding within the mangroves of Brgy Layag, Zamboanga City and resulted to a
great damage to the said vehicles. Despite the return fire coming from the enemy, the fight
was able to perform the strafing and aerial reconnaissance mission at the area. The fight
was also instrumental in helping the Naval Forces in their offensive activity at the area by
giving the exact enemy location. Unmindful of their personal safety, these brave men risked
their lives and had successfully completed the strafing and reconnaissance mission which
resulted to the great advancement of government troops over the enemy. By this
achievement and performance coupled with gallantry and self sacrifice, these pilots and
crew earned distinct credit not only for themselves and the Philippine Air Force but for the
Armed Forces of the Philippines.

coastal waters of Zamboanga City and Basilan thereby maintaining security in the vicinity
and preventing the movement of the enemies using the sea. Likewise, he sacrificed his
personal safety by closing in on the mangrove areas where land forces are engaged with
the enemies, in order to reinforce friendly forces firepower, prevent enemy reinforcements
and deny their escape. As Commander, Naval Task Group Mariqui, he effectively
employed his surface assets composed of four (4) Multi-purpose Attack Craft (MPAC), one
(1) Cyclone Vessel, three (3) Patrol Gunboats and two (2) teams from Naval Special
Operations Group to secure the coastal waters of Zamboanga City and control all
movements of watercrafts in the area. Through his leadership and expertise in naval
operation, the unit was able to conduct 25 naval patrol missions and 6 combat support
missions and inspected a total of 72 watercraft during the operation, there by contributing
to the resolution of the Zamboanga Crisis. By this achievement, subject Officer earned
lasting honor not only for himself and the Philippine Navy but also for the Armed Forces of
the Philippines.

contributed largely to the overall accomplishment of the mission in containing,
constricting, and defeating the rebel forces and to the eventual ending of the
Zambaoanga Crisis with 161 rescued hostages, 142 enemy surrendered and 165
enemy killed and the recovery of 171 firearms to include numerous EID and
thousands of ammunitions and a peaceful and liberated Zamboanga City. By these
achievements, CAPTAIN FAJARDO earned lasting honor not only for himself and
the Philippine Navy but also for the Armed Forces of the Philippines.

for acts of heroism involving risk of life as the pilot of UH-IH No. 759 that conducted search
and rescue operations at the lahar affected areas of Manibuag, Pasig, Porac and Bacolor,
Pampanga on 16 September 1995. Upon receipt of urgent call from the Regional Disaster
Coordinating Center, FIRST LIEUTENANT DEPAY JR., together with his co-pilot and two
enlisted men, proceeded to the areas to pick up residents stuck in floodwaters caused by
strong rains. Unmindful of his personal safety, he led the group and braved the poor
visibility and inclement weather to save precious lives. Despite limited capability due to lack
of rescue equipment, he successfully brought to safety several persons who were swept
away by the rampaging lahar. By this remarkable achievement, FIRST LIEUTENANT
DEPAY JR earned lasting honor not only for himself and the Philippine Air Force but also
for the Armed Forces of the Philippines.

for acts of heroism involving risk of life as Team Leader and members of the Medical Team
of the Special Operations task Group Special Operations Command, Philippine Army
during the height of the Zamboanga Crisis perpetrated by armed faction of the Moro
national Liberation Front under Habier Malik from 10-23 September 2013. Unmindful of
imminent danger, MAJ CANDELARIA and his men push forward from Casualty Collection
Point to Pick-Up Point and most often in the engagement areas despite heavy firefight to
extricate combat casualties in the frontline that saved the lives of 78 wounded soldiers.
With high state of discipline and professionalism, MAJ CANDELARIA and his team
performed Tactical Combat Casualty Care not only to government forces, civilian bystander
hit by crossfire, hostages and even the wounded enemies. Their profound concern for the
lives of others bespeak well on their devotion to duty and indubitable servitude to the
Filipino people that significantly contributed to the success of the over-all effort to resolve
Zamboanga Crisis that resulted to 180 Enemy Killed (BC), the Capture of 279 RME,
Surrender of 24 RME and the recovery of 240 assorted Firearms that likewise led to the
rescue of 195 Hostages. By these acts MAJ CANDELARIA and his men earned lasting
honor not only for themselves and their unit but also for the Armed Forces of the
Philippines.

his personal safety, MAJOR HALIPA also initiated contacts with concerned MNLF
Commanders not to interfere, reinforce or sympathize with the RMEs but instead,stay

for meritorious achievements in aerial flight and for sustained operational activities in the T-
334 aircraft from the time it was decommissioned on 7 April 1994. During the eruption of Mt
Pinatubo, only three T-33As were evacuated to ABAB. The others were damaged beyond
repair, yet the Squadron Commander, MGEN JOSE TONY S VILLARETE, his Ex-O, CAPT
DANIEL M PENA and the other instructor pilots were able to check out four new students.
Another remarkable achievement was the completion of the upgrade to Element Lead
Training of three PAF officers. They continued to fly such sorties as Air Defense Training
missions, reconnaissance and persuasion flights all over the country. Due to dwindling
resources, the aircraft grounded, accumulating only 64 sorties to fly them after months of
being discurrent, with no one to check them out for recurrency, since not a single pilot in
the Wing was their current in the aircraft at that time. By these achievements, these officers
earned lasting credit for themselves and the Philippine Air Force.

for heroism achievement as members of the 2nd Scout Ranger Company, 1st Scout Ranger
Battalion, First Scout Ranger Regiment, Special Operations Command, Philippine Army
during an encounter with undetermined number of armed elements of the Moro National
Liberation Front – Misuari Faction at vicinity Target 7 and 8 Sta Barbara, Zamboanga City
from 23 to 24 September 2013. Unmindful of their personal safety, Sergeant Viajedor and
his section established defensive positions at the occupied objective in order to prevent
enemy forces from breaking away from their final strongholds and subsequently trapped for
the final push of the main effort. The troops detected some enemy while trying to flee from
the tightening noose of the government forces carting with them one (1) unidentified killed
rebel. Subsequently the defensive position to give orders and making sure that each
approaches or exits of the enemy are manned and continuously being monitors, resulted to
one (1) killed. Likewise, the troops encountered another enemy who tried to breakthrough
but was repelled by the troops leaving behind one (1) enemy dead. The seizure of the
enemy strongholds has made a devastating blow on the enemy, cutting them off from any
opportunity to escape and relocate. This greatly contributed in the over-all plan to
decimate any remnants of the enemy to reach havoc elsewhere. By this heroic deed,
Sergeant Viajedor and his section earned honor not only for themselves but also for the
Armed Forces of the Philippines.

Meanwhile, a wreath made of Sampaguita has its tendrils tied in the ancient
Filipino script K encased in a red triangle at the center base. There are flowers laid

on the outer boarder of the inner circle arching 2/3 upwards supporting a royal blue
enameled scroll with the embossed Latin words “Veritas” (Truth) and Patria
(Country) to the left and right, respectively. The color red in the triangle symbolizes
courage and patriotism and the royal blue freedom, and the ancient Filipino script K
stands for “Kapanalig.”
Then, parallel to the center circle are embracing seven white lines directly
below the uplifted Sampaguita leaves. This Sampaguita wreath means recognition
for service and achievement, while the seven white lines represent the Seven
Pillars of CMO namely: Military Values Education, Military Livelihood enhancement,
Civic Action, Community Relations, Public Information, Psychological Operations,
Research and Special Studies.
On the lowest center portion of the medal are the AFP superimposed over a
platform and connected to the left and right sides with laurel leaves two in a row,
one on top of the other, with the topmost longer in length bordered by six toothed
gears for each of the left and right flank of the medal. The laurel wreath signifies
honor and greatness, enclosed by the AFP triangle, and then the gear with a total
of twelve teeth represent the whole year of dedication and service.
At the center of the topmost portion of the medal on the left oblique is a
Tambuli angled downwards the right of a feathered pen superimposed crossing
from the right oblique descending to the left of the tambuli and feathered pen
symbolizes the historical implements of the medium.
b. RIBBON. The ribbon has alternating colors of gold and royal blue stripes
distributed from center to near edges, which shall be in drab green. The gold stripes
shall number eight (8) and the royal blue seven (7) and drab green two (2) in all.

July 1986, as revised, the award MILITARY COMMENDATION MEDAL is hereby awarded

for exemplary, efficiency, loyalty, and devotion to duty as personnel of the Office of the
Assistant Chief of unified Command Staff for Military Operations, OU7, EMC who
successfully spearhead the Photo and Static Display at the Abreeza mall, Davao City from
10 to 12 June 2013 in connection with the national celebration of the 115th Philippine
Independence. With dedication to duty, LTC AQUINO and five (5) others initiated
coordination with Ayala mall Management and the different units within the Eastern
Mindanao Command Area of Responsibility to be able to implement appropriate activity
amidst the limited time and resources. With this, every participant were able to showcased
their respective capabilities through static displays and photo exhibits and in turn resulted
to an interactive participation that draws interested civilians to join the military organizations
through the Recruitment Information Booth installed in the area of activity. Moreover, the
event was also envisioned for cohesiveness among stakeholders like the Civil Society
Organizations to promote awareness on Peace issues. By these achievements, above
named Officers and Enlisted Personnel earned distinct credit, not only for themselves, but
also for the Armed Forces of the Philippines.

for meritorious and invaluable services rendered as the Chief, Operations Branch,
Office of the Secretary Joint Staff from 16 November 2010 to 16 March 2011. Imbued
with deep sense of responsibility and professional competence, MAJOR CALLANTA
carries his tasks with utmost enthusiasm and zeal. He assisted the Secretary Joint
Staff in carrying the latter’s disposition of very significant communications requiring
sensitive decisions. He formulated the Communications Management Information
System that facilitated the inventory and accounting of communications reaching the
OSJS. He also crafted and formulated policies that improved the flow of
communications and correspondences coming from and going to the OCSAFP,
OVCSAFP. MAJOR MALLANTA maintained the prescribed administrative and
operational record of all operational activities of the OSJS, thereby ensuring the
accomplishment of tasks of OSJS. By these achievements, MAJOR MALLANTA has
earned credit not only for himself and the office which he belonged to, but for the
Armed Forces of the Philippines as well.

for conspicuous gallantry at the risk of life beyond the call of duty as leader of a 5-
man team of CHDF that encountered 30 heavily armed lawless element that raided a
village in the hinterlands of Basilan and looted the people of their belongings and
valuables on 13 April 1990. Upon receiving report of the raid, Mr Dela Cruz led his
four companions in intercepting the lawless elements. A fierce firefight ensued and
amidst hail of bullets, he maneuvered to a vantage position under covering fire from
his comrades. With grim determination, he moved from position to another with his
firearms spewing death. After the smoke of gunfire dissipated, enemies were found
dead on the scene of encounter while the other were believed wounded as shown by
the traces of blood along the way of withdrawal. By this gallant act, Mr Dela Cruz
distinguished himself in combat, thereby earning lasting honor for himself and his
unit.

for conspicuous courage and gallantry in action as member of the Badiangan CAA
Patrol Base, 2nd Surigao del Sur CAA Company, under the operational control of the
29th Infantry Battalion, 4th Infantry Division, Philippine Army, during an encounter with
four fully armed terrorist dissidents of the Front Committee 77 at vicinity Sitio
Badiangan on 4 January 1995. While negotiating a trail towards the long abandoned
farm, CAA Bandoy, with one CAA and the two civilian volunteers incidentally
chanced upon a farmer who informed them of the presence of four fully armed
dissidents about 100 meters away from their position, believed to be facilitators for
the passage of enemy supplies coming from Barangay Aras-asan, to the suspected
enemy lair in the hinter areas. With no time to inform their detachment about the
situation, CAA Bandoy opted to confront the armed dissidents despite the risk of their
lives for being outnumbered. With courage, he took advantage of the thick
undergrowth and tactically maneuvered towards the enemy position. A fierce firefight
ensued between them for ten minutes. He directed his companions to give him cover
as he moved from one place to another to be able to penetrate the enemy’s location.
Once in a vantage position, he and his companions delivered heavy volume of fires
towards the enemy, prompting the latter to withdraw, leaving behind two killed
comrades and one M16 rifle. One of the enemy fatalities was later identified as the
alleged Finance Officer of the Front Committee 77. By this display of courage, CAA
BANDOY earned lasting honor not only for himself but also for the military
organization.

Support Command, Philippine Army during the conduct of various Humanitarian Assistance
Disaster Response Team for the victim of Super Typhoon Yolanda which barreled through
most of the areas of Visayas with an initial total of Five Thousand Two Hundred Twenty One
(5,221) beneficiaries or One Hundred Seventeen (117) Minor Surgery; Four Thousand Four
Hundred Twenty Eight (4,428) Consultation and Treatment; and Six Hundred Seventy Six
(676) people processed for Evacuation to Metro Manila. The overall unit accomplishment
indicative of unwavering commitment of the subject Officers and Enlisted Personnel in
bringing the services of the government were all significant and tangible contributions to the
typhoon victims of Tacloban, Leyte and the rest of the Visayas Region. By these
achievements, above-named Officers and Enlisted Personnel have immeasurably contributed
to the overall mission of the Philippine Army in winning the hearts and minds of the people
thereby earning distinct credit and honor not only for themselves but also for the Armed
Forces of the Philippines as well.

3) Shovel And Pick – During the late 19th Century, this basic icon was
adopted by the Army of the Philippines Revolution as a part of their military
insignia. These tools are the fundamental construction implements of the first
