JUDICIAL AFFIDAVIT
I, Mylyn Cajucom, Filipino, of legal age, single and a resident of Imelda Valley, Palayan
City, Nueva Ecija, after having been duly sworn in accordance with law, do hereby depose and say;
That I am the same and identical person who caused for the execution of this Judicial
Affidavit under the supervision and at the law office of the undersigned counsel with the name and
address stated below;

That I had given my answers on the questions below stated with full consciousness and I
did so under oath, and that, I may face criminal liability for any false testimony or perjury;

That the following are the questions asked by the undersigned counsel with corresponding
answers which I had given.

QUESTION AND ANSWERS

1. Q: Please state your name and other incidentals.
A: I am MYLYN CAJUCOM, of legal age, Filipino, single and a resident No. 55 Brgy Imelda
Valley,Palayan City Nueva Ecija.

2. Q: Do you know personally the defendant “FRITZ”?

A: No maam.

3. Q: How did you meet the defendant?

A: He approached and joined our table at AURA Bar in Cabanatuan maam.
 4. Q: How about your friend Benidict, is he related with Mr. “FRITZ”?
A: I don’t know maam.

5. Q: Where were you on October 15, 2018, at around 8:00 in the evening?
A: We were at Aura Bar maam; we were celebrating my birthday.

6. Q: What were you doing in that Birthday party?

A: We were eating and drinking maam.

7. Q: Where you drunk during that party?

A: A little maam.

8. Q. Did you observe something unusual between Benidict and Fritz that night?
A: None maam. They were merely talking and drinking their own bottle of beer.

9. Q: What time did you leave the party?

A: Around 11 p.m maam.

10. Q: How did you leave the party?

A: My boyfriend picked me up maam.

11. Q: Do you have anything else to say Ms. Cajucom?

A: I have nothing more to say maam.
