    You have been selected to participate in a study of parents of young children.

    Who is conducting the study? The University of Wisconsin-Extension is doing


    the study, with the cooperation of your local hospital.

    Do I have to participate? No, of course not. But we hope you will. Your
    answers will help us make decisions about how best to serve parents like yourself.

    How long will this take? This questionnaire only takes about 5 to 10 minutes to
    complete.

    Who will see my answers? Only the research staff. Your answers will be strictly
    confidential. We don’t even ask for your name.

    OK, what do I do? Fill in the questionnaire as quickly and honestly as you can.
