In order to facilitate the prompt submission of the Form 48 (OTR), all PSDSs and School
Heads are directed to follow the attached Flow Chart of Activities on the checking of the said form
with its attachments.
Deadline for submission of DTR from the District Office/Secondary Schools to this Office is
