Statement of Need, Problem Statement


Attendance significantly contributes to academic performance of students.

(Romer, 1993) Hence, attendance is closely monitored in both private and public

schools. In fact, public schools allotted a form for the checking of attendance of the

students which is known as School Form 2 (SF2) or the Monthly Attendance

Students’ Log. Data from this form will be used to identify students who are subject

for home visitation. Meaning, these are the students who are having frequent

absences or students who continuously fail to be present within five school days of the

week. Home Visitation is part of the Drop-Out Reduction Program of the Department

of Education. As the teachers of Hulo Integrated School try to stay devoted to their

profession and conduct home visitation, an incident where a male teacher was

stabbed on the shoulder during the home visitation forced them to suspend the activity.

What happened served as an enlightenment to the teachers that there is no existing

policy, guidelines and protocols in doing home visitation. This appeared problematic

as it hinders the progress of extending aid to students who are near to dropping from

school. It also postpones the finalization of list of students to be encoded in the

Learners Information System (LIS). Worse, sentiments from the family of the

concerned students about their perceived absenteeism are yet to be confirmed. These

only show that home visitation is a must and there should be a followed guidelines to

prevent the recurrence of unexpected events. This may not be only true to Hulo

Integrated School but even to other schools in the division of Mandaluyong.

Spearheading the making of the home visitation guidelines will not only benefit one
school but could be of great help to all public schools in Mandaluyong and even to the

neighbouring cities.

Goals, Objectives and Outcomes


The proponent aims to draft a Home Visitation Guidelines for future

implementation. The said guidelines should be amenable to all parties concerned so

that full support to the program will be highly expected. It shall bring confidence and

security to the school personnel involved Families will renew their partnership with

school as they receive updates about the status and progress of their children. Most

importantly, concerned students will be given another chance to pursue their studies

and be guided in dealing with their personal struggles in life. Once more, they will be

reminded that school is their second home and their teachers are their second parents.

Plan of Work, Narrative or Project Description


The following steps should be taken to ensure success of the proposed project.

a. The proponent shall draft home visitation guidelines.

b. Drafted home visitation guidelines shall be amenable to both parties. Hence,

meetings with the concerned authorities should be held.

c. Advisers shall identify students who are living in Barangay Hulo and other

barangays.

d. Once, the guidelines is finalized, an orientation for both parties shall take

place to make sure mutual understanding.

e. Home Visitation as guided by the finalized guidelines shall commenced on


agreed schedule.

f. After the first implementation, an evaluation will be conducted so that further

revision will be considered.

Evaluation
One purpose why there should be followed guidelines in conducting home

visitation is to secure the teachers and empower them to continue implementing the

program. The success and validity of the drafted guidelines will be evident if teachers

are more confident to brave small and narrow pathways located in slum areas just to

make sure that they will be able to connect-first hand with the family of their students.

It is also considered as a success when the parents realize that the work of the school

is not only limited within the four corners of the classroom but it is extended to an extra

mile, trying to reach out by bringing school to their home. But above all, having

students come to school again, motivated to complete a week of school is what will

determine that it is a job well done. That only reflects that students have felt how

welcomed are they despite being gone for a while, how they are being taken care of

by their second family and how hope for a brighter future has once started. These are

the situations stated in the evaluation form, which the parent will accomplish, based

on what will take place after the conduct of home visitation. Accomplished evaluation

form shall be reported to the guidance counsellor and school principal. Accomplished

evaluation form could also be a tool for the revision of the guidelines if necessary.
HULO INTEGRATED SCHOOL AND BARANGAY HULO MEMORANDUM OF

AGREEMENT ON THE IMPLEMENTATION OF HOME VISITATION

I. Purpose of the Agreement

This Memorandum of Agreement (MOA) sets out the terms wherein Hulo

Integrated School and Barangay Hulo will work together in implementing an

efficient and effective Home Visitation Program to provide follow up on

students who fail to complete a week or five school days in a week. Through

this, opportunities shall be extended to students who would still want to

continue with their studies. Threatening concerns about students are the

priority reasons in conducting home visitation. However, it is not limited to

problematic reasons. Home Visitation is also encouraged to commend high

performing students and could be randomly done.

II. Roles and Responsibilities

Responsibilities of Hulo Integrated School:

Before Home Visitation

1. Hulo Integrated School, specifically the adviser of the concerned student

should provide enough written communications informing the parents or

guardian of the performance of the student before proceeding to home

visitation.

2. Hulo Integrated School shall identify students who are subject for home

visitation based on supporting school forms primarily SF 1 (Learner’s

Student Profile) and SF 2 (Monthly Attendance)

3. Hulo Integrated School shall schedule home visitation. Suspension of

classes can be maximized for home visitation, but holidays shall not be

consumed. However, the schedule should be confidential within both parties


as it should appear as a surprise homevisit to ensure validity of the conduct

of the home visitation.

4. Hulo Integrated School shall send a group of teachers composed of all

subject area teachers, adviser and at least one of the following; head

teacher, grade level chairperson, coordinator, counselor or assistant

principal.

During Home Visitation

1. Representatives of Hulo Integrated School shall strictly follow the following

in order:

a. Ask for proof of identification to ensure the accuracy of the visited home and

family.

b. Upon identification, the adviser shall assist the parent or guardian in

accomplishing the Home Visitation form.

c. Purpose of home visitation shall be stated clear to the parent or guardian.

2. Home visitation shall not exceed 45 minutes upon arriving.

3. Evaluation form shall be accomplished by the parent or guardian to further

improve the program.

4. Representatives from Hulo Integrated School shall not initiate any form of

violence that will cause unlikely disturbance to the whole community.

Note: Home Visitation shall progress if the student is present except for the

following reasons: student is too far from home (willfully left, at the province or

abroad and other related reasons and circumstances)


After Home Visitation

1. Hulo Integrated School shall observe confidentiality in all accessed

information.

2. Post conference right after the conduct of home visitation should be done in

the school.

3. Reports and documentation should be furnished in three copies (guidance

counselor, barangay secretary and class adviser) and shall be submitted three

days after the visitation.

Responsibilities of Barangay Hulo:

Before Home Visitation

1. Representative from Barangay Hulo shall confirm the residency of the

identified students.

2. Representative from Barangay Hulo shall confirm availability based on the

given schedule by the school.

3. Barangay Hulo shall make at least 2-3 “barangay tanods” available to assist

the teachers conducting home visitation.

4. Barangay hall shall remain available should there be a need to access it.

5. Initiative to suspend home visitation shall be granted, should there be any

current concerns and issues in the community that could be a threat to both

parties.
During Home Visitation

1. Barangay Hulo shall take charge in maintaining peace and order while home

visitation is ongoing.

2. A representative from Barangay Hulo shall accompany the teacher inside the

house to serve as a witness of the implementation of the program.

3. Representatives from Barangay Hulo shall stay as long as the other party is

within the area of visitation.

4. Representatives from Barangay Hulo shall not initiate any form of violence

that will cause unlikely disturbance to the whole community.

After Home Visitation

1. If necessary, representatives of Barangay Hulo shall accompany the other

party going back to the school.

2. Same representatives of Barangay Hulo shall remain available for further

steps to be taken based from the result of the home visitation.

3. Representatives of Barangay Hulo shall accomplish evaluation forms to

conclude and better the program for succeeding home visitation.

III. No Collection Policy

No Collection Policy is strictly observed in implementing home visitation

program. However, kind gestures such as providing snacks shall only be provided and

is only limited between both parties.


IV. Duration of the Agreement

This agreement will be in effect throughout the school year and may be updated

at any time through written agreement of each party.

V. Signature of Both Parties

If the terms of this Memorandum of Agreement are acceptable, please sign and

date both copies of this agreement letter.
