Dear Ma'am/Sir,

The purpose of this letter is to request for some important documents for me to
complete my SSS Maternity Benefit Requirements. I became Alfamart's employee at
AMI Marinig Perez (Cabuyao Laguna) for more than 1year. ( Nov.2016 – Dec.2017).
When I am 3months pregnant I already file my SSS MAT. 1 to H.R. Department but I
resign before I gave birth and I have to continue the process on my own to get my
maternity benefit.

So I have to request for this following documents.

-Certificate of Employment/Separation
-Certificate of Non-cash advancement on maternity benefit
-Valid L501 (specimen signature card)

I will be very thankful if you make immediate and positive response for my concern
because this is so important for me and my baby.
