A long, long time ago, in a scary island of Samar, there lived a 65 year old female.

She is
hypertensive, non-diabetic, non-asthmatic and no history of heredofamilial disorders. She claimed to have
lactose intolerance but with no other allergies to food and drugs. She is rumored to be a very strict teacher
but she is married to a retired government employee and have three very healthy children.
In 1972, she undergone appendectomy.
In 1996, she had endocervical polypectomy.
In 2001, she had cholecystectomy and all of the 3 mentioned operation had an unremarkable
hospital stay.
In 2004, she was diagnosed to have a multinodular goiter with cystic degeneration.
Because of these events in her life, people started talking and believing that she was cursed. Later
on, people slowly started avoiding her.
She is at present menopause with regular checkup and pap smear with her OB-Gynecologist and
claimed to have unremarkable findings with present medications include: Levothyroxine, Amlodipine (5
mg tablet once a day) and a multivitamin.
Eventually the rumors died down, until 2 weeks ago she had watery, profuse diarrhea. No fever,
abdominal pain and any other manifestations noted. She took Loperamide (Imodium) 3x a day and
consumed Gatorade 5 bottles per day with fortunately temporary relief.
But then a week ago, condition recurred now associated with fever, self-medicated with
Metronidazole 500 mg tablet 1 tablet 3x a day and Paracetamol 500 mg tablet 1 tablet every 4 hrs. She
continued drinking Gatorade. No other manifestations mentioned. She claimed to have temporary relief
again.
Several hours prior to consult, condition recurred now associated with generalized abdominal
pain and vomiting and took Hyoscine Butylbromide (Buscopan) but this time afforded no relief. People
saw her seen by an internist/healer.
Vital Signs: BP= 140/100 mmHg, HR= 70 beats per minute, RR= 17 breaths per minute and
Temperature = 38.5C and claimed to have 10-20% weight loss.
Did she accidentally harmed nuno sa punso? Did an engkanto fell in love with her? Or did she fail
in her class a child of a mambabarang?
